APPLICATION FOR PROBATION

Comes now the accused ____________________________, hereby


applies for probation under the provisions of Presidential Decree No. 968,
as amended, and declares that:

1. Accused ____________________ is married, and a resident of


Barangay Tuban, Sablayan, Occidental Mindoro;

2. That he was convicted of the crime of Violation of Republic Act No.


10591, otherwise known as the Act Providing for a Comprehensive
Law on Firearms and Ammunition and Providing Penalties for
Violations Thereof, and sentenced to suffer “an imprisonment of
STRAIGHT SIX (6) YEARS”;

3. That accused has not begun to serve the term of imprisonment


imposed upon him;

4. That accused wishes to avail himself of the benefits granted by the


Presidential Decree No. 968, otherwise known as the Probation Law
of 1976, as amended;

5. That the crime for which accused was convicted is not an offense
against the security of the state;

6. That accused has not been once on probation under the provisions of
Presidential Decree No. 968, as amended;

7. That accused undertakes to comply with such terms and conditions


as this Honorable Court may impose if his petition for probation will
be granted.

PRAYER

WHEREFORE, premises considered, the accused most respectfully


prays that this application be favorably acted upon.
