AFFIDAVIT OF LOSS

I, ELEONOR DINLAYAN., Filipino, of legal age, and a resident of Purok-1, Patin-ay,


Prosperidad, Agusan del Sur, after having been sworn in accordance with law, hereby depose
and state THAT:

1. That on July 27, 2019, I went to Cebuana Lhulier-San Francisco branch to pawn a jewelry
particulary described as “Lady’s Ring w/ IS 18k, 2.8 grams”;

2. In the said transaction, Cebuana Lhulier appraised the jewelry and granted a loan in the amount of
P5,700, and I was issued a corresponding receipt thereof (PT#50607SG03517)

3. Since lastweek, I have been searching the said receipt because I intended to redeem my jewelry, but I
could not find it anymore;

4. I exercised due diligence in locating my Receipt and have notified its loss to Cebuana San Francisco;

5. My efforts to locate my Official Receipt (OR) have failed since until now the same has not been
found, leaving me with no other recourse, but to consider it lost beyond recovery; and

6. I attest to the truth of the foregoing statements.
