AFFIDAVIT OF LOSS

I, Lailanie E. Panganoron, Filipino, single and a resident of Purok 5, Patin-ay,


Prosperidad, Agusan del Sur, after having been sworn in accordance with law, hereby depose
and state THAT:

1. I am a student of Surigao del Sur State University (SDSSU) by reason of which I was issued
School ID No. 1007171;

2. Sometime October 2016, while I was at school I may have misplaced the said ID;

3. I exercised due diligence in locating my ID and have reported its loss to the proper school
authorities;

4. My efforts to locate my ID have failed since until now the same has not been found, leaving me
with no other recourse but to request and process for the issuance of a replacement ID, for which
purpose this affidavit is executed; and

5. I attest to the truth of the foregoing statements.

IN WITNESS WHEREOF, I hereunto affix my signature this 4 th day of May 2017 at Patin-ay,
Prosperidad, Agusan del Sur, Philippines.
