AFFIDAVIT OF LATE REGISTRATION OF BIRTH


I, AAAAAAAAAAAA, of legal age, Filipino, single and a resident of Cebu City, Cebu,
after having been duly sworn to in accordance with law, depose and state THAT:

1. I am the father of BBBBBBBBBBBBBBB;

2. He was born on 17 February 2020 at Cebu City;

3.  I was delayed in registering his birth for the reason that I reported back for work
and due to this COVID19 Pandemic I was not able to process the registration of his
birth;

4. I, together with my live in partner, the child’s mother, CCCCCCCCCCCCCCC,


would like to apply for the delayed registration of the birth of our son;

5. I am executing this affidavit to attest to the truth and veracity of the foregoing facts
and in compliance with the requirements of the Philippine Civil Registrar.
