Assessment Nursing Diagnosis Planning Intervention Rationale Evaluation

The nursing diagnosis was disturbed body image related to dissatisfaction with appearance from illness. Short term goals were for the patient to understand body changes and long term was relief from anxiety and adaptation to actual and altered body image. The plan was to assess mental and physical factors relating to illness, evaluate knowledge and anxiety about the situation, listen to comments and responses, assist correcting underlying problems, encourage communication, set limits on maladaptive behavior and identify positive behaviors, and assist identifying actual changes. The rationale was to determine causative factors, acceptance of the situation, understand perspectives, promote healing and adaptation, and address any misperceptions.
