AFFIDAVIT OF PARENTAL CONSENT


TO TRAVEL ABROAD

I, ROLLIE RIZMA MALULAN, Filipino, of legal age and with


residence address at Victoria, Tago, Surigao del Sur, after having been
sworn to in accordance with law, do hereby allege – THAT:

1. I am the biological father of ROCHE SUAZO MALULAN, 22 years


old, and who is going for a trip abroad;

2. Being her parent, I am giving my full consent to my daughter to


travel abroad;

3. For this purpose, we will defray the necessary expenses that my


child may incur, so that she will not be a burden to the state or
place of destination at any time during her trip abroad;

4. Further, no litigation in wherever court in the Philippines to which


my child is a party or has been involved; and

5. I am executing this affidavit to attest to the veracity of the


foregoing and to allow my child to accompany her grandparents to
Vietnam.

IN WITNESS WHEREOF, I have hereunto set my hand this 26 th day


of June 2014, in Tandag, Surigao del Sur.

I, PETER JOHN DEXTER M. LUGA, Filipino, of legal age and with


residence address at Purok Maganda, Bag-ong Lungsod, Tandag City,
Surigao del Sur, after having been sworn to in accordance with law, do
hereby allege – THAT:

1. I am one of the compulsory heirs of the late Domingo Garcia Luga,


who died on August 27, 2006;

2. Being as such, I am one of the claimants of whatever benefits due


to our deceased father as a bona fide member of GSIS, as well
those which are due to us, being his surviving legal heirs;

3. However, the filing of our claim was delayed due to reasons which
are beyond our control;

4. One of said reasons is our delayed receipt of the “death claim” and
“survivorship benefits” from the GSIS, which is deemed as our
clearance for us to commence our claim regarding our father’s
terminal leave;

5. I want to declare herein that we already received and enjoyed the


benefits mentioned above, thus, we are now claiming the terminal
leave of our father;

6. I am executing this affidavit to attest to the veracity of the


foregoing and to request for an immediate release of our claim
pertaining to the terminal leave of our deceased father.
