1. Name of Cold Storage


2. Address & Contact No. of Applicant
3. Capacity of Cold Storage
4. Application duly filled up in prescribed format A, B & C.
5. Application form duly attested by concerned DHO.
6. Project report of multichambered cold storage with complete financial details
(Backward and Forward linkage)
7. Necessary document of registration of company/individual firm.
8. Copy of up to date registration (License) of cold storage installation.
9. Copy of Land Deed
10. Copy of recent land revenue receipt.
11. Sanction letter of bank
12. NOC of Pollution Control Board
13. Certificate of Fire Officer (District Fire Office)
14. Evidence of genuine electric connection.
15. Copy of insurance policy.
16. Date, Month, Year of construction/completion of cold storage/expansion of
cold storage.
17. Date/Year of application applied for Subsidy.
18. Appraisal report (economical technical feasibility report) of concerned bank.
19. Address proof of applicant & other partners (PAN card/Voter ID card/
Passport/ UID etc.)
20. Letter of concerned bank claiming subsidy.
21. Details of installation of plants & machineries.
22. Area & crop to be covered by the cold storage.
23. Project proposal submission in five set.
24. Provision of Generator set.
25. Utilisation certificate of the bank concerned.
26. NOC from NHB & NABARD regarding nonpayment of subsidy to the
applicant.
