1.0 PURPOSE

The procedure provides the general guidelines for compilation of project final
documentation and the sequence of their content.

2.0 SCOPE

The procedures outline the CONTRACTOR's responsibility for preparation and


compilation of the Project Final Documentation for Jacket, Topsides, Onshore
Facilities, HUC and Installation works.

5.0 PROCEDURE

5.1 GENERAL
The final documentation is required for reference during initial start-up of
the facilities and as a day to day reference during operations and
maintenance. It also provides information necessary for the future check of
equipment performance or for planning of works, plant expansion or
upgrading. It shall be prepared for quick and easy reference for all users.

5.1.1 TOPSIDES and ONSHORE FACILITIES

The final documentation for the above facilities shall comprise the
following documents with their corresponding Volume numbers:

Note (#): For PCC/EPCC/EPCIC projects, all other volumes

waiting for HUC activities and related documents.
Note (##): Shiploose Master List is not applicable for onshore

5.1.2 JACKET

The final documentation of the Jacket shall comprise the


following documents with their corresponding Volume numbers:

Note:
a) If EQUIPMENT DOSSIER is required for the Jacket
compilation, CONTRACTOR shall prepare approved
documentation for Volume 1 – Equipment Dossier compilation
same as of Topsides.

b) CLOSE-OUT REPORT and SHIPLOOSE MASTER LIST


shall form part of the Final Documentation.

c) The contents of final documentation for Jacket (i.e. QA/QC


DOCUMENTATION, MATERIAL DOCUMENTATION,
AS-BUILT DRAWINGS and PROJECT SPECIFIC
PROCEDURE) shall be similar to that of the Topside except
for the Volume numbering.

d) The number of books required for each particular Volume shall


be based on the amount of documents to be filed.

5.1.3 HOOK UP & COMMISSIONING (HUC)

The final documentation of the HUC shall comprise the following


documents:

i. VOLUME 1 – EQUIPMENT DOSSIER

Hook-Up CONTRACTOR (HUC) shall provide Final


Documentation for distribution and to be submitted in hardcopies
and softcopy version as per contract requirements.
For AS BUILT Drawing, CONTRACTOR shall update all redline
marked-up drawing into softcopy ACAD latest version and to be
approve by PCSB prior to producing hardcopy for compilation.

5.1.4 OFFSHORE INSTALLATION

The final documentation of the OIC shall comprise the following


documents:

i. VOLUME 1 – AS-BUILT DRAWING


ii. VOLUME 2 – PROJECT REPORT
iii. VOLUME 3 – SURVEY REPORT
iv. VOLUME 4 – PIPELINE PRE-COMMISSIONING
REPORT
v. VOLUME 5 – QA/QC DOCUMENTATION

Offshore Installation CONTRACTOR (OIC) shall provide Final


Documentation for distribution and to be submitted in hardcopies
and softcopy version as per contract requirements.
For AS BUILT Drawing, CONTRACTOR shall update all redline
marked-up drawing into softcopy ACAD latest version and to be
approved by PCSB prior to producing hardcopy for compilation.

5.1.5 There are no designated numbers for CLOSE-OUT REPORT,


SHIPLOOSE MASTER LIST and NDT RADIOGRAPHIC FILM.

5.1.6 When certain item headings within each Volume Table of Content
are not applicable, it shall be indicated as “NOT APPLICABLE”
in the Volume Table of Content (VTOC).

5.1.7 The number of books required for each particular Volume shall be
based on the amount of documents to be filed. The book
numbering shall be in numerical sequence based on the total
number of books for the particular Volume e.g. Volume 1
comprises of three books therefore, the book numbering shall be
Book 1 of 3, Book 2 of 3 and Book 3 of 3.

5.2 PREPARATION OF FINAL DOCUMENTATION TABLE OF


CONTENT

The final documentation table of content shall be prepared by


CONTRACTOR prior commencement of the compilation work. For
standardization purposes, CONTRACTOR is required to prepare the main
and volume table of content as per format given in the Appendix I and II
respectively. The draft table of content shall be submitted to PMT and
CARIGALI for approval.
The main table of content covering the entire set of volume shall be placed
in the front page of every book. The volume table of content shall be
placed immediately after the main table of content and each section shall

be separately tabbed and labeled. The volume table of content (VTOC)


shall be placed accordingly to the volume of the Final Documentation i.e.
VTOC for Volume 1 is to be placed only in Volume 1.

5.3 CONTENTS OF FINAL DOCUMENTATION

CONTRACTOR shall be responsible to ensure that the contents of final


documentation are complete and contain the contents as listed below. If in
the opinion of the CONTRACTOR, that some other documents which are
not listed in the table of contents but it is necessary for future references,
then it shall be included as part of the final documentation. However, any
addition to or deletion from this list shall be subjected to CARIGALI's
approval. The content of each volume and sequence shall strictly adhere to
the following listing.

5.3.1 VOLUME 1 – EQUIPMENT DOSSIER

Equipment Dossier is a compilation of the final vendor data or


documents for CONTRACTOR (both bulk materials and
equipment items) and CARIGALI supplied items. The
arrangement of the equipment shall adhere to the Appendix V
(Final Documentation Manual Appendix 1). For each equipment,
the compilation of the document shall follow the following
sequence. (Sample VTOC as per Appendix V Figure I)

5.3.3 VOLUME 3 – MATERIAL DOCUMENTATION

The Material Documentation is a compilation of all documents


related to the material and equipment procured by CONTRACTOR
and CARIGALI. The Material Documentation shall comprise of
the following information:

i. Purchase Order
ii. Source Inspection Report
iii. Material Inspection & Receiving Record
iv. Certificates (Mill Cert., Manufacturing Cert.., Third
Party Inspection Cert.., Test Cert...)
v. Material Deficiency Report or Overage, Shortage &
Damages Report (OS&D)

For the certificates requirement, vendors / suppliers are to provide


original certificates. If not, the master copy must be stamped

“CERTIFIED TRUE COPY” and signed by a third party


certification agency.
For item ii, iii & v, all reports shall be typewritten for the
standardization with other volumes in Final Documentation
Compilation.

5.3.4 VOLUME 4 – TESTING & PRE-COMMISSIONING


DOCUMENTATION

The Testing and Pre-commissioning Documentation is a


compilation of Detailed Pre-commissioning Procedure, check
sheets overviews, testing and pre-commissioning check sheets
which are grouped by the system and other reports in the following
sequence:
For PCC, EPCC or EPCIC Project, consolidation of the approved
documents shall be combined together from pre-commissioning
until completion of HUC activities. The title of the volumes shall
be “Testing, Pre-Commissioning, Hook up And Commissioning
Documentation”.

5.3.5 VOLUME 5 – DESIGN DATA

5.3.5.1 There are two (2) types of design data documents for
the final documentation compilation for CARIGALI
project.

a. Conventional Project
Report for the Design Data compilation of the design calculations
performed by CONTRACTOR during construction stage for the
work performance and compliance for completion of the WORKS.
CONTRACTOR shall prepare the required discipline documents

only for the purpose of compilation (whichever applicable). The


Design Data shall cover the following:

b. PCC, EPCC, EPCIC Project


Compilation of the final Design Data and Report
which is prepared by DESIGN CONSULTANT during
the design stage are compiled together with the design
calculations performed by CONTRACTOR during
construction stage for the work performance and
compliance for completion of the fabrication works,
and HUC activities. The title of the volumes shall be
“Design Data and Report”.

5.3.5.2 Weight Control Report from Design Consultant and


Contractor shall be compiled together at the end of the
Design Data Volume under separate index. However,
only the final WCR during fabrication activities will be
included in the black binder compilation.

In the absence of Design Data Volume (for Jacket


compilation), the Weight Control Report shall be
compiled as item, in normal binder.

5.3.6 VOLUME 6 – AS-BUILT DRAWING

As-Built Drawings is inclusive of all the drawings submitted by


CARIGALI and all other drawings developed and prepared by
CONTRACTOR which are submitted for CARIGALI approval
and information during the execution of the CONTRACT. All As-
Built Drawings shall be grouped as per the following for each
Module, where applicable.
For all drawings standard format, numbering and revisions,
CONTRACTOR shall refer to PTS 20.097 Addendum Item 9. All
discipline As-Built Drawings master copy shall be stamp “AS-
BUILT” in RED ink. (Sample VTOC as per Appendix V Figure VI)

5.3.7 PROJECT SPECIFIC PROCEDURE

Project Specific Procedure is a compilation of all approved


procedure related to the project scope of works during
construction / fabrication stage, installation, commissioning and
load-out activities. All approval procedures compiled into binders

5.3.8 START-UP AND OPERATING MANUAL

Start-up and Operating Manual is a document describing the


guidance and instruction for day-to-day and safe operation of the
facilities/platform. Operating Manual shall be written in user-
friendly language and terminology. Master copy (for offshore)
shall be printed on glossy paper while the remaining set shall be
printed on normal paper.

5.3.9 CHANGE ORDER DOCUMENTATION

Change order documentation is a compilation of all approved


change order of the project amendment from the contract
requirement as agreed and approved by the CONTRACTOR and
PCSB Project Team.

5.3.10 CLOSE-OUT REPORT

The Close-Out Report is a report which describes the execution


and overall project status of the WORKS. The report shall
comprise the following information:

The Shiploose Master List is a document of all items which are to


be transported loose to offshore or CARIGALI Supply Base such
as shiploose list, packing list, drawings and other technical
information of all the shiploose items for offshore installation,
hook-up and commissioning. The Shiploose Master List shall

5.3.11.2 Packing List


A compilation of all lists such as packing list for
Equipment Dossier, As-Built Drawing, Testing and Pre-
commissioning Documentation etc. which have been
transported together with the Jacket or Topsides to
offshore.

5.3.11.3 Shiploose Tie-In Drawing


A compilation of the approved drawing of all material
or items which to be installed or hooked-up offshore
including punchlist items.

5.3.11.4 Shiploose Layout Plan


A sketch of the plan showing the location of all
shiploose material on the Jacket /Topsides or
transportation barge.

5.3.12 NDT RADIOGRAPHIC FILMS

Image of weld and material of applicable piping, structural and


mechanical items. Compilation of the NDT film shall be as per the
Lines system. CONTRACTOR shall prepare and compile master
listing index for PCSB references.

5.4 As per fabrication contract document requirements, CONTRACTOR shall


prepare five (5) items consigned for offshore works. The items are as
follows:
i. Equipment Dossier
ii. Testing & Pre-Commissioning Documentation
iii. As-Built Drawing
iv. Shiploose Master List
v. Onshore Punchlist
CONTRACTOR shall consolidate and compile all the above items into
temporary binders prior to load out and sail away.

6.0 SPECIAL REQUIREMENTS

6.1 As the work progresses, all documents for Final Documentation shall be
compiled progressively and submitted for CARIGALI’s review.

6.1.1 Final Documentation for all volumes shall be compiled into black
binder as per Appendix V: Final Documentation Manual
requirements.

6.1.2 PCSB to approve black binder requirement prior to purchase by


CONTRACTOR.

6.1.3 The black binder specification must abide to the following:


a. To use six (6) reverts on piano type hinges
b. Hard cover chip board with 5mm thickness
c. Black color quality if vinyl with gold print matt hot stamping
d. Binder shall be standard to US Ring Binders (fully nickel
plated or stainless steel) with piano-type hinges, center locking
post and three (3) binding post.

6.1.4 To use PVC paper divider with six (6) banks white color type
130gms as a main separator in all data compilation. For sub-
indexes, CONTRACTOR shall use green color A4 paper for the
first three sub-indexes, and pink color A4 paper for package items
document content as a separator. (Refer to Appendix V Item 4.0)

6.2 CONTRACTOR to stamp “AS-BUILT” in RED ink on top right hand


corner of the title block for each approved package items as-built drawings
submitted by vendor/supplier.

6.3 CONTRACTOR to stamp “FINAL” in RED ink on the master copy


compilation for each volumes and books. Types of documents for the final
stamping are as follows:
i. FINAL stamping on the front page of the approved MTOC and VTOC
only
ii. FINAL stamping on the front page of approved package items
documentation only.
iii. FINAL stamping shall be stamped on the top right hand corner
regardless of paper size and orientation either vertical or horizontal.

6.4 CONTRACTOR shall deliver a full complete set HUC/Installation of


offshore redline marking drawings in proper binding to CARIGALI after
completion of works. This requirement is also required for the onshore
facilities project.

6.5 All documents for master copy (regardless of Volume) must be of original
or good master print without any history. Final approved documents/data
submitted by vendor(s) must be clean print and is accompanied by
approved cover letter.

6.6 CONTRACTOR shall be responsible to send all the Final Documentation


by using galvanized metal box size W 17” x L 21” x H 14” to the
respective parties as per distribution matrix in Appendix IV. Details of the
addresses will be given by CARIGALI.

7.0 RECORDS
7.1 CONTRACTOR’s Project Manager shall be responsible to ensure this
procedure is made available to the respective personnel for each discipline
section in the Project Team.

1.0 GENERAL

The Specification outlines the requirements for the Final Documentation for the fabrication phase of
offshore structures or similar facilities, which shall be made up of the following:

CONTRACTOR is required to furnish the documents as a ready reference for all important work
structures, equipment and items installed during the project. It is intended to assist in the initial start-up
of the plant and to provide quick reference in subsequent operations and maintenance. It also provides
information necessary for future checks of equipment performance, planning of works, plant expansion
and redesign work.

CONTRACTOR shall ensure that purchase orders clearly state the vendor data required to enable
compliance with the Final Documentation.

When any package unit is supplied by a vendor or subcontractor, the supplier is required to submit vendor
data for equipment to CONTRACTOR for inclusion in the Final Documentation.

The Final Documentation provided by CONTRACTOR shall cover both COMPANY's and
CONTRACTOR's supplied items.

1.1 DEFINITION

For the purpose of this manual the following definition shall hold:

Shall and Should - the word "shall" is to be understood as mandatory and the word "should" as strongly
recommended to comply with the requirements of this manual.

CONTRACTOR - Is the party, which comes out all or part of the design, engineering, procurement,
construction, commissioning or management of a project or operation of a facility.

COMPANY - Is the party, which initiates the project and ultimately pays for its design and
construction. The company will generally specify the technical requirements. The company may also
include an agent or consultant authorized to act for, and on behalf of, the company.

All pages and binders shall be standard A4 (210 x 297 mm) and (245 x 310 mm) size
respectively. Text shall be hack on white legibly reproduced. Blue print type text copy is not
acceptable.

All vendor drawings larger than 297 x 420 mm shall be folded and inserted in individual 210 x 297 mm clear
pliable plastic pockets which shall have standard three-ring side punching for insertion in the
binders. The requisition and sheet number, Flow Plan Symbol Number or item number and the
number of the drawing shall be written on the face of each containing pocket.

As-built drawings are required in A1 (full size) or A3 (297x420 mm) sizes. Only A3 size as-built drawings
shall be put in binder. Drawing hinders shall have three binding posts. All as-built drawings done by
CONTRACTOR shall be black and white photocopy quality and legible. Drawing index shall appear
in front of each section and section tabs are to be furnished showing drawing serial number and category.

Binders shall be black color quality vinyl material covered with print Matt hot stamping gold block lettering
as per Appendix 3. Bookbinders shall have 80 mm minimum capacity with 50 mm free extendibility
with full length "piano-type" hinges, a center locking post, and 3 binding posts for papers. Binders
shall be in accordance or equivalent to:

a. Earl Gray Bookbinding Co. (1220 Maple Ave. Los Angeles 90015, California), 3” model GK
binder: or

b. American Loose Leaf Corp. (180 Main Ave. Califton. N.J.) Herculex type binder or

c. Johnson Cover Co. (Houston Texas) Model F798

CONTRACTOR shall fill binders only to a maximum of 100-mm thickness. The remaining 25 mm
extendibility is to provide space lot, COMPANY future additions.

Each set of documentation shall be given a sequential number. The Volume number shall be printed on
the binder cover and backbone and Book number only on the backbone.

The requirement for the binder details shall be standard to US Ring Binders (fully nickel-plated) with
piano-type hinges, center locking post and three (3) binding post.

Volume and Book numbers shall be Arabic numerals.

3.0 CONTENTS

All documentation’s are to be typewritten to the maximum extent possible. In the event that more than
one model are described on a documentation, the data for actual model used shall be identified with
appropriate 'arrows’ and ‘boxes' to highlight that information.

Sequence of insertion of data shall conform to Appendix 1.

Book shall be numbered consecutively so that there will be no duplication or omission.

An Equipment Information Sheet (see Appendix 2) is to be compiled to account for all major items.
Only fixed and semi-permanent items are to be listed. Generally, these include all items with tag
numbers assigned plus furniture, power tools and kitchen accessories but not hand tools/hand
kitchen accessories. Only individual panels or "packages" are to be listed rather than tagged PI's, PCV's
etc.

The content of each document is briefly described below:

3.1 EQUIPMENT DOSSIER

The following information is required for each equipment

a. A summarized information (Equipment Information Sheet - Appendix 2) to include all


important data concerning the equipment such as serial number, model number, tag number,
vendor and local agent contact address and telephone numbers, and other relevant
information, which shall form the front page for the documentation for that equipment.

b. Completed engineering data sheets and test / calculated performance data in accordance with
applicable specifications, technical data, performance test reports and design calculation.

c. Outline drawings, special construction details, electrical wiring and inter-connection diagrams.

d. All installation operation and maintenance instruction manuals, catalogues, recommended


spare parts list, where available, include spare pails reorder/replacement frequency and
recommended maintenance schedule summary,

e. Complete manufacturer's parts list, sectional and blow-up drawings with parts identification
Number, brand name, nomenclatures and brand identification number for his equipment.

f. Actual Factory Acceptance Test (FAT) Procedure and Report

g. Certification Records and Warranty Documents

h. Material and Inspection Report

3.2 QA/QC DOCUMENTATION

QA/QC Documentation shall consist of all records and reports on field inspection and testing of
materials and products performed by CONTRACTOR. Documents to be included shall be limited to
non-destructive testing records, welders’ qualification test and records, field inspection i.e. visual
inspections on fit-ups, welding, dimensional check and records, coating qualifications and production test
and hydrostatic test reports.

3.3 MATERIAL DOCUMENTATION

Material Documentation shall consists of all documents pertaining to the procurement and handling
of materials such as purchase orders, source inspection reports, materials identifications, certificates of
material and test reports, materials deficiency reports and materials receiving inspection reports.

3.4 TESTING & PRE-COMMISSIONING DOCUMENTATION

Testing & Pre-commissioning Documentation shall include all documents pertaining to the testing
and pre-commissioning such as Detailed Pre-commissioning Procedures and check sheets.

3.5 DESIGN DATA

Design Data is a compilation of Structural Analysis, Pipe Stress Analysis, Process calculations, electrical
and instrumentation calculations etc.

3.6 AS -BUILT DRAWINGS

As-Built Drawing is a compilation of all drawings submitted by COMPANY and all other drawings
developed and prepared by CONTRACTOR for COMPANY approval and information during the
execution of the CONTRACT,

All those drawings that are defined in the CONTRACT are required to be updated to show the As-
Built condition at the end of the project.

All drawings shall be clearly stamped "AS-BUILT" with red color and signed by the appropriate
CONTRACTOR's representative.

Drawings that are to be updated to the As-Built state shall be all drawings, which the
CONTRACTOR submits for COMPANY's approval and information during the execution of the
CONTRACT.

As-built mark-ups are to be made available to COMPANY upon COMPANY's acceptance of the
facilities. CONTRACTOR also to provide COMPANY with complete set binding of redlines drawing mark-
ups for reference use.

4.0 INDEXING

A main table of contents covering the entire set of final documentation shall be included in the front
page of each binder or book. In addition, each section shall have separately tabbed, immediately
following the main table of contents, the specific VOLUME Table of Contents.

Each VOLUME shall have its own contents listed separately.

Each VOLUME shall be divided into Group, Section, Sub-Section, etc., by color-coded dividers and
index tabs as per the following:

(a) Main Table of Contents - blue lab


(b) Volume Contents - red tab
(c) Group - clear tab
(d) Section & Sub-Section - clear tab
(e) Sub-index - A4 Green and Pink color simili paper

the location of where the title may he found in the Final Documentation. The index shall be inserted in the
rear of every book.

All dividers shall PVC paper divider Type six (6) Banks white colour with 130 gms for the Main Table
of Contents, Volume Table of Contents and Group dividers. Separator for sub-index shall be green
color A4 and pink color A4 paper size for the package items document content.

All dividers shall have tabs. The tabs shall be an integral part of the dividers and indexed on both sides.
Self-adhesive plastic tabs with slip-in inserts shall not be used. Tabs shall be staggered such that 5 or
6 tabs will be in view at all times.

Samples of dividers with tabs shall be presented to COMPANY Site Representative for approval.
Suitability shall be judged or conformance to the specifications and grade of material for the dividers.

6.0 LATE DISTRIBUTION OF DATA

Project changes, which are made in the late stages of execution, may make it necessary to distribute data
after the Final Documentation has been distributed. In this event, the data are to be separated, 3-hole punched, and
each document is to be clearly marked (volume, section, and page) for easy insertion into the documentation set.
New indices, manila pockets, and/or additional binders are to be prepared and forwarded to COMPANY's
Construction Manager for distribution.
