DBT Case Formulation Template - Brief Format

This DBT case conceptualization document outlines the key elements to address for a client, including treatment targets, applying Linehan's biosocial model, identifying dialectical dilemmas, and listing common chain analyses. The targets cover suicidal behaviors, treatment interfering behaviors, and quality of life. The biosocial model examines biological and social factors. Dialectical dilemmas involve themes of active-passivity, unrelenting crisis vs inhibited grieving, and emotional vulnerability vs self-invalidation. Chain analyses map how behaviors can link to suicidal thoughts.
