II. FAMILY HISTORY

a. Genogram

b. Maternal & paternal grand lineages

Mary’s father is the eldest of 11 siblings. All of his siblings have

stable job. Her paternal grandfather was an army and was fond of
participating in vices such as ‘’cockfight and ‘’tong-its’’. At the age of 52 he
was working as a security guard in Malaybalay City. Eventually, he was
killed due to stabbing incident while on duty. Her paternal grandmother
however died with unknown disease. She was a loving mother but was too
loving that she spoiled her children especially Mary’s father even after he
became married.

On the maternal side, Marry’s mother is the 2 nd daughter of 11

siblings. Her maternal grandfather died due to hypertension at the age of
87 while her maternal grandmother was also died at the age of 95 with
unknown disease. They were strict in terms of house rules but they were
described as loving parents. They never hit their children; they will just
scold them and reprimand them on their mistakes.

III. PARENTS

1. Father
 Mary’s father was a disciplinarian type of person. He is 74 years old
a senior citizen. He only finished high school. He was hard-working as a
father. He was a part of Philippine constabulary and work as an army in
malaybalay but during martial law time he was decided to resign at the
age of 45. Before He was managing their farm planting corn, sugarcane
and other vegetables to provide and support his family. He was also
described as short-tempered and frequently drinks and smokes. He would
go home drunk but he never hurt her wife and children. He enjoyed going
with his friends. He verbalized that “ usahay kay gapanguha ko ug kwarta
ka misis para ma-nari”. Then that’s the reason they are quarreling with her
wife in front of our children but he never had an affair to other women. He
said that he love and respect her wife and his children.

2. Mother

Mary’s mother is a retired teacher. She is 73 years old a senior

citizen also. She was currently in their house and taking care of her
husband and grandchildren. She disciplines her children by just
reprimanding them. She would reprimand her husband when he gets too
abusive. She has a high blood pressure and she take medication that
prescribed by the doctor like losartan. She wants to take care of her
daughter in happy home but sad to say she is not allowed because of his
age. She loves her family especially to her children. She wants her
children have stable in life.

4. Siblings

Mary’s is the 2nd of 6 siblings. She has an elder sister. A married

woman who is currently working as a teacher. She takes care of Mary at
happy home sometimes. Her younger sister is also a teacher, she is also
the one who takes care of Mary if her elder sister is busy at work, she lives
with her husband and children. His younger brother died due to heart
failure and stroke at the age 44 while his younger sister also died due to
infantile beri-beri for four days only. Her youngest sister is also married,
finished her second year and is currently working as an OFW in Dubai.

Mary was a good to his siblings. At the age of 16 Mary got pregnant and
gave birth to a healthy baby boy. She was not able to get married to the
 father of his son because her mother wanted her to finish her studies and
also she was young at that time of her pregnancy. The father of his son is
willing to marry her but her mother disagreed.

IV. PERSONAL HISTORY OF CLIENT

a. Prenatal & perinatal history & birth

Mary, the client, said that she carried herself well and didn't experience
any bumps, falls, and other accidents during pregnancy. She was a
teenager when she got pregnant. It was difficult for her to be pregnant at a
very young age. She had prenatal check-ups every month with the guide
of her mother. She never experienced any sickness during her pregnancy,
as stated by her mother. She was careful regarding her pregnancy. Mary
took vitamins, supplements and eats green and nutritious vegetables
during her pregnancy.

b. Infancy
According to the mother of the client, she experienced a complete nine
months of pregnancy. The mother gave birth to Mary in the hospital
through normal delivery without difficulty attended by a doctor on May 4,
1970, in Don Carlos, Bukidnon. She was breastfed for one year and was
raised by a nanny. Mary completed her immunizations according to her
mother. The mother didn't notice any abnormalities in raising her child,
Mary.

c. Toddler
Mary was breastfed for one year by her mother after birth. The feeding
pattern during pregnancy was normal. According to her mother, her child
cuddles hugs her every time when she breastfed her. Mary was a sweet
child as stated by her mother. She showed positive nocturnal enuresis and
completed her immunizations as stated by her mother.
She was taken care of by a nanny because her mom was a teacher and
her dad works in the military. Her first tooth came out when she was 8
months old. She started walking when she was one year old. The toilet
training started when she was 3 years old and it was mostly done by her
mother. According to the mother, thumb sucking ends when she was 4
years old.
As she grows, she gets along and plays with her brothers and sisters. She
likes playing in front of their house with the guide of her parents.

d. Preschool
 During preschool, she was viewed to be a sweet, loving, and friendly child.
The mother stated that she was very helpful in house chores. At her young
age, Mary started doing some simple house chores together with her
brothers and sisters. Her parents love her very much because she was
helpful at a young age. She loves playing with dolls with her friends.
According to her mother, she started being an achiever in her pre-school.

e. School age
During school-age, she was friendly and gets along with the other
students at school. She had many friends, and her mother stated that she
was a talkative student. Mary likes studying very much. There were lots of
subjects that she likes. She was a consistent honor student since pre-
school.

f. Adolescence
During adolescence, she was still very friendly. She had many female
friends. She likes going out at night with her friends and have fun. In this
stage, she met a guy the same as her age, and she got pregnant at the
age of 16. During her pregnancy, she was not allowed to talk to her
boyfriend. Her mother was strict and Mary was separated from the father
of her child for a long time. After her pregnancy, she was able to continue
her studies. She was still an achiever after having a baby. She takes care
of her baby with her mother's guidance. According to the mother, she
adopted Mary's baby under her name because Mary at that time was still
young.

g. Young Adult to Middle Adult

h. Intellectual History
She studied well and focused on her chosen course that's why her grades
were fair. Her grades were great to pass the subjects and course. She
was an achiever from pre-school until college. According to her mother,
she experienced "nabughat" while studying and she was not able to eat on
time because of her busy schedule. She graduated the degree of Bachelor
of Science in Elementary Education at Central Mindanao University. She
took the board exam and passed. She was assigned at San Fernando for
two months with her friend.
i. SocialHistory
The client was an active teacher in her school. She was very dedicated
and goes along with his colleagues, friends, students, and family. She acts
accordingly and was fond of dealing with her co-teachers. Later part of her
life when she went at San Fernando to teach, she had a good relationship
with her female friend. According to her mother, everything started when
she was assigned at San Fernando. Her daughter told her that she saw a
big snake that was about to strike her. She got very nervous and scared
after that thing happened. She got very sick when she went back home
from her working place. She became socially withdrawn, suspicious,
always nervous, and later became aggressive. Her mother stated that she
was holding a knife while staring blankly. She got very aggressive and
keeps on shouting at everyone around her.

j. Psychosocial History
She was 14 years old when she started to become aware of sex. She likes
wearing cute dresses, as stated by her mother. She was able to
experience a great childhood experience. She was 16 years old when she
tried having sex with her boyfriend and got pregnant with her first child.
Mary gave birth to her son, Rommel.
The client was a single mother. However, she did everything for her son.
According to the mother of the client, her daughter shares a passionate
love with her child.
