AFFIDAVIT OF CHANGE OF ADDRESS

I, SHIRLY ANGELIN GRACE EASWARADHAS, of legal


age, Indian national, single, and a resident of Lakandula St.,
Brgy. 15-B, Davao City, after having been duly sworn in
accordance with law, hereby depose and say that:

1. I am the person who caused the execution of this


affidavit;

2. I am an applicant for an Alien Certificate of Registration


(ACR) bearing card no. G0000053091 before the Bureau
of Immigration, Davao City;

3. On February 22, 2020, I moved-in to my new apartment


at Unit 221, Linmar Towers, Porras St., cor. Lakandula
St., Brgy. 15-B, Davao City, from my former address at
215 2/F DMEMS Dorm., Jacinto cor. Juan Luna St., Davao
City;

4. I am informing the Bureau of Immigration, Davao City


and any other concerned agencies that my new resident
address is located at Unit 221, Linmar Towers, Porras
St., cor. Lakandula St., Brgy. 15-B, Davao City;

5. I am executing this Affidavit to attest to the truth and


veracity of all the foregoing and to use the same for
whatever legal purpose it may serve best.
