Independent: After nursing intervention, the patient was able to: - Assess the patient's ability to communicate through speech and nonverbal methods. Monitor for facial expressions, pointing, crying etc. Encourage speech when possible. Maintain eye contact and speak clearly, slowly when communicating. Refer to speech therapist if needed. Rationale: Speech disorders are common in patients with conditions like Klinefelter syndrome. Identifying communication problems helps create a care plan. Nonverbal communication shows feelings. Clarity in speech promotes response. Speech therapists help with disorders. Evaluation: The nursing intervention helped the patient exhibit minimal frustration with speech attempts. The patient was able to communicate needs verbally or nonverbally as required
