2. PRE-WORK PRECAUTIONS (To be checked by Authorized Issuer of the Permit) Confirm by stating “YES” of “NO”

Has the obvious combustible hazards, i.e. dust, flammable gases, etc. been identified
Has the surrounding area been made safe, at least 10 m from hot work?
Has the area been made safe for hot work performed from an elevated position – 15m from hot work?
Has the correct fire extinguisher been determined and is it available?
Is there proper ventilation in the area?
Has the necessary welding shield been put in place?
Are emergency and evacuation procedures established and understood?
Has equipment been locked out, if necessary?
