Subcontractor shall develop a specific HSE Plan for the Project as per scope of work, which contain
but not limited:
1. Objective & Target
2. Scope of Work
3. HSE Responsibility & Job Description for project, (HSE organization, Top Level Management
responsibility on HSE)
4. Man power planning & CV (HSE Coordinator + safety officer ratio 1:50 worker)
5. HSE Program, HSE daily meeting, HSE weekly meeting, Equipment & tools inspection ,
Emergency drill, Training, safety induction, etc.
6. HSE Communication program (toolbox meeting, banner, safety signage, safety information board
etc)
7. Construction activities plan & method statement
8. PPE mandatory for the project refer to PPE Standart of PP EPC. (safety glass, long sleeves,
2 HSE Plan trousers, safety shoes, welding cap combined with helmet, harness double lanyard, etc) BID
9. Detail JSA
10. Detail Risk Assessment
11. HSE Reporting system (daily & weekly)
12. Emergency Respone Plan
13. List of fisrt aid equipment and fire prevention (APAB, fire blanket, First aid kit, emergency eye

14. Environmental Management Plan (Non Hazardous & Hazardous Waste Management - include
the number of waste container for non hazardous waste, temporary storage, and final disposal,
Spillage Prevention, Waste Water and Drainage Management & Air Pollution Control) etc.
15. HSE Budget shall be considered by subcontractors
Final HSE Plan shall be submitted by Subcontractor after award and shall be approved by
Contractor prior to activities

All lifting equipment shall be have valid Local Government certificate or the third party that certified
KAN and all lifting works will require a lifting plan for approval prior to performing the lift. In the lift is
over than 20 metric tons or classified as critical (exceeding 75 % of crane capacity chart, High
pressure pipe, or high voltage power lines etc), the subcontractors/vendors will submit detailed
rigging plan with all applicable supporting calculations to Main Contractor or Owner for approval

Lifting gear (Shackle, Sling, Webbing, Spreader Beam/Bar, Masterling, Chainblock, Lever Block)
must manufactured with ASME B30 Series Standard and have inspected by third parties proven by
the "surat uji riksa" from PJK3. All lifting gear shall be inspected regulary. Broken/Substandard
lifting gear shall removed from site immediately.
Subcontractor shall submit certificate of personnel (as per scope of work) for Project, which contain
but not limited :
1. HSE Coordinator with AK3U certificate with min 5 years of experience in construction.
2. HSE officer with minimun 3 years of experience. Must be provided with ratio 1:50 worker or refer
to the risk assesment per scope.

material storage area, B3 waste TPS, permanent generator set and so on.
