Student’s Affidavit
 I solemnly declare that my father/guardian permitted me to take admission in Govt. Jinnah
Islamia College, Sialkot. I accept, being student of this institution, that I’ll abide by in letter and
spirit all the rules related to academic activities.
 I further pledge that I haven’t enrolled in any other institution for an undergraduate degree
programme and college can cancel my enrollment, if proved at any stage.
 I also assure that the Principal of college can expel me if I remain unable to perform according to
minimum desired results in academics.
 I shall fulfill 75% college attendance record otherwise my admission to Board/University should
not be sent.
 I also pledge that the institution can rusticate me immediately if I’m involved in violation of
discipline, student’s provocation for strike and breaching the law. The principal is further
authorized to not entertain my re-admission request.

Father/Guardian’s Affidavit

all the dues i.e. fees, fine etc. shall pay on time.
 I further declare, if my son/daughter is involved in some illegal eruption against college
authorities or he/she is involved in some riots; I’ll bear the physical loss of college. Meanwhile,
I’ll not sue to any court of law if he/she is expelled from college permanently on disciplinary
charges.
 If the provided information in the admission form proved bogus at any stage, the college is
authorized to expel my son/daughter. Further, college authorities are liable to proceed against
him/her under the given public law.
 If my son/daughter is involved in some political or unlawful activity, my all sympathies shall
stand with college.
 I shall be regular in my contact with college. If I don’t respond to the call of college at the need of
hour, I authorize to college to take one-sided decision against my son/daughter and I’ll accept this
decision.
 If my son/daughter doesn’t meet minimum criteria desired academic results/attendance record,
I’ll neither object on the decision of college nor sue any court of law.

Required Documents for Form Submission
The copies of following documents will have to attach with admission form.
 Three (3) attested photocopies of result card of Matriculation (SCC)
 Three (3) attested photocopies of result card of Intermediate (HSSC) or Part-I
 Three (3) attested photocopies of Character Certificate of the last institution attended.
 Three (3) attested photocopies of Candidate’s CNIC/Form-Bay
 Original Equivalence certificate(s) issued by Inter Board Committee of Chairmen (IBCC),
Islamabad/Lahore in case of O/A Level(s).
 Three (3) attested photocopies of Candidate’s Domicile
 Three (3) attested photocopies of Father/Guardian’s CNIC
 Four (4) unattested Student’s Passport size Photos with Blue Background
 Rs.100/- only Judicial Paper (‫ )اشٹام پیپر‬issued in the name of candidate or Father/Guardian.

Note: At the time of final approval of the admission/interview, the candidate shall be
required to personally visit the college in accompany with his/her parents/guardian and
original testimonials for necessary verification etc. The admission will be finalized after
the completion of all required formalities.
