AFFIDAVIT OF RELEASE OF CO-MAKERSHIP

I, EUSEBIO C. CLAVISILLAS, Filipino, of legal age, and resident Siloy, Calamba,


Misamis Occidental, under oath, hereby depose and say that:

1. I am a member of Misamis Occidental Employees Cooperative (MOEMCO)


being a an employee of Provincial Engineers Office and severe my service
at the said office last February 29, 2021;

2. Being a member of the said MOEMCO, I was a co-maker of the many co-
members who applied for loan particularly to SHOLEM KABANLIT, DANTE
JUMAWAN, PETRUS GATELA, JULIUS PADERANGA, PHILIP JOHN
CALAMBA, WILSON BELINGAN, EFREN PADERANGA and JOSELITO
CALOG;

3. Since I retired already from service, I withdraw my capital stock in the


said MOEMCO and consequently be released from my obligation as a co-
maker of the abovementioned names;

4. I am already free from any obligations to MOEMCO nor indebted to any


amount;

5. I am executing this affidavit to attest to the truth of the foregoing facts


and for whatever legal purpose this may serve me.
