I declare that I have read, fully understood and accept the Withdrawal Policy section of relevant Examination Handbook(s). I understand
that only the examination fee(s) for the examination paper(s) specified in Part 2 of this form will be refunded and an administration fee per
examination per examination paper/module per examination mode per examination session will be deducted from the examination fee(s).
I hereby apply for a withdrawal for the examination paper(s) specified in Part 2 of this form and I understand that once the Hong Kong
Securities and Investment Institute has received my application form, I cannot withdraw my application for a withdrawal and my enrolment
for the corresponding examination paper(s) will be cancelled. I understand that the refund will be payable to me in Hong Kong dollars
only.

本人謹此聲明已閱讀、清楚明白並同意接納相關考試手冊內之取消報名政策項。本人明白香港證券及投資學會只會退回本表格第二部份所
填寫之試卷的考試費用，並會於退回考試費用時，就每個考試每份試卷每個考試模式每個考試試期扣除相關的行政費用。本人現申請取消
報考本表格第二部份所填寫之試卷的考試，並明白香港證券及投資學會於收到本人的申請表後，本人之取消報考申請將不能撤回，上述相
關試卷之報名申請亦會被取消。本人明白退款只會以港幣支付予本人。
