AFFIDAVIT OF SITE INSPECTION

I, LINO L. BALANQUIT, SR., of legal age, married, Filipino, and resident


of San Juan corner Solana Streets, Brgy. 7, Poblacion, Pambujan, Northern Samar,
after having sworn to according to law hereby depose and state:

1. That I am the Proprietor/Manager of J Square Builders and Construction


Supply, a duly licensed construction firm under existing laws of the
Republic of the Philippines.

2. That I made an Ocular Site Inspection of the project with Solicitation No.
2021-04-0001- CONSTRUCTION OF BOBON FIRE STATION, BOBON,
NORTHERN SAMAR.

3. That this affidavit is executed in support of my bid for the said project which
will be implemented by the BUREAU OF FIRE PROTECTION- REGION
VIII.

IN WITNESS WHEREOF, I hereto affixed my hands this 4 th day of May


2021 at Catarman, Northern Samar.
