TERMINAL REPORT FOR THE MONTH OF JUNE 2020
I. INTRODUCTION
Pursuant to DepEd Order No. 12, s. 2020 which is the Adoption of the Basic Education
Learning Continuity Plan for School Year 2020-2021 In the Light of The Covid-19 Public Health
Emergency, the San Jose Pili National High School – La Purisima Annex strictly followed the
mandated policies in order to help prevent the spread of corona virus amongst students, parents,
and other stakeholders. In line with this, the school also adopt the Basic Education-Learning
Continuity Plan in order to continuously provide students the necessary learning they need to
have even this time of pandemic.
II. ACTIVITIES UNDERTAKEN
These following activities are undertaken in the month of JUNE 2020:
School Preparation for the opening of classes by October
Everybody in all sectors is affected by the pandemic, yet the Education Sector did
not lose hope in providing knowledge to the students. So in this month, teachers were
actively preparing the school through accepting enrolees and giving Learning Enrolment
Survey Forms enabling the teaching personnel to be ready on the mode of teaching that will
be used when the opening of classes come.
At the end of every week, the school records-in-charge kept on updating the
enrolees, and other teachers were busily giving LESF to the community of La Purisima,
following the strict health standard protocols, wearing face mask, face shield, and putting
alcohol every time.
The Grade 7 teachers were able to conduct the Phil IRI Test that helped the
teachers to identify the level of reading comprehension of Grade 7 enrolees.
The school also asked the helped of the Barangay Council, GPTA Officers, and
4P’s Members to conduct a rabus in preparation of the school’s class opening. Since
vacation had gone through, it is indeed remarkable that the school vicinity had been
cleaned which brought a little good atmosphere in the school surrounding. This rabus was
done following proper health protocols to avoid whatever sickness.
Providing face mask to parents and students
Experiencing this worldwide pandemic brought so much hassle to the lives of
every person. This is the time where everybody is obliged to have their face mask on
every time they are outside their houses. So, because of this the school came up with the
idea of providing free facemask; portable sewing machines were purchased to create
facemask, and 60 facemask were also donated which had been distributed to the students
and parents of SJPNHS-La Purisima.
BE-LCP (Most Essential Learning Competencies (MELCs)
In preparation to the opening of classes, the teachers of SJPNHS-La Purisima
Annex started to download the necessary learning materials to be used by the students
for school year 2020-2021 that is hereby adopted from the BE-LCP mandated by the
Department of Education. It is in accordance with DepEd’s MELCs, wherein experts
had expertly edited the contents of every learning area in all levels that will be suited to
the students in this time of pandemic. Students were not bombarded with so much
activity, instead they were only provided of the Most Essential learning they need during
this time.
III. PROBLEMS ENCOUNTERED
No person had experienced a perfectly time and event of his life, that is why encountering
problems are part in growing up in learning life lessons. In view of this, some of the things
encountered during the conduct of the said activities are the lack of cloth needed to sew for a
face mask; another is the lack of available materials needed for disinfection, such as: alcohols,
spray container for alcohol used when there are visitors, liquid detergents and bleach for the foot
bath.
For the module preparation, some of the downloaded materials were lacking of subjects
and bond paper also run out during the printing of Learner’s Enrolment Survey Form.
IV. RECOMMENDATION
To suffice the problems encountered, the school asked for a student or parents who will
volunteer to sew face mask in their houses. The teachers also provided their own disinfecting
materials that will be used for the meantime, as well as printing materials like bond paper.
No hard job can be surpassed when everyone is in cooperation with what is being carefully
delegated.

The pandemic has created a lot of change in our daily lives, for public schools the challenge
now lies in continuing education. As a result, education has changed dramatically, the distinctive
rise of e-learning and modular distance learning, whereby teaching is undertaken remotely and
on digital platforms.
As the educators gear up with the new normal set-up in this corona virus pandemic, this is
the appropriate time to use our discomfort to forge ahead in the new educational set-up. The
SJPNHS-La Purisima Annex participated in the different trainings and programs provided by the
Department of Education and other sponsoring agencies to cope up and make the teachers
prepared on the opening of school year 2020-2021 under the new normal.
II. ACTIVITIES UNDERTAKEN
The following activities were undertaken on the month of July 2020:
Enrolment
The remote enrolment scheme in SJPNHS-La Purisima continues using Facebook,
text messages, e-mail and drop box at the barangay hall of La Purisima. The teachers
contacted the parents to make an interview on filling-up of the Learner Enrolment and
Survey Form.
Brigada Eskwela 2020 under the New Normal
The annual Brigada Eskwela which aimed to prepare for the opening of classes has
evolved into a brand new packaging of cooperation between the educators and the parents
amid the coronavirus pandemic under the joint theme: “Pagpapanatili ng Bayanihan
Tungo sa Kalidad na Edukasyon para sa kabataan.”. The Brigada Eskwela in SJPNHS-
La Purisima provided a scheduled date for its stakeholders who are willing to help in
cleaning the school with strictly following the minimum health standards and those who
are willing to donate any materials that can used to the new normal class.
3 day I-SMILE Webinar nSeries
The 3-day webinar hosted by the SDO CamSur was conducted on July 13, 17 and
20, 2020. It tackles a board range topics just like the Basic Education –Learning
Continuity Plan,
Online Training of the Provision of Psychological first aid to learners

2020. The seminar workshop lets the participant engage in exploring the Google
classroom; how to create a Google classroom, inviting students, how to assign task, how
to attached learning materials, and evaluating student answers. It was an interactive
workshop that gave knowledge to the teachers on how to use Google Classroom Properly.
III. PROBLEMS ENCOUNTERED
The problems that have been encountered during attending the webinars are: not every one
of us has stable internet connection and even others encountered poor signal connections that
caused delay and miscommunication to the speakers of the said event. In addition, poor audio
quality and delays in joining the webinars were also experienced.
IV. RECOMMENDATION
Based on the problems observed it will be more helpful if the teachers have stable internet
connections at their home to fully participate and understand what the speakers are discussing.
Also, make a sound-check every speaker 30 minutes before the webinars, this way, they may
have time to address any issues with audio quality in advance.

TERMINAL REPORT FOR THE MONTH OF SEPTEMBER 2020
V. INTRODUCTION
Professional development requires continues education. The demands of K-12 curriculum
adhere to fill the gap of the face to face to new normal setting. Teachers must be equipped with
new knowledge and skills to ease the difficulties faced by teachers in this new set up in this time
of pandemic.
Furthermore, SJPNHS-La Purisima Extension held series of engagement activities to suffice
the demand of the new normal education. By this, teachers are able to enlighten by the new ideas
that are suitable in this time of education and provide better ideas to the students that can support
their modular distance learning of education.
VI. ACTIVITIES UNDERTAKEN
These following activities are undertaken in the month of September 2020:
Preparation of Modules
At the very first day of September teachers are all busy in preparing for the
opening of classes. We first arranged our faculty rooms for us to have a place where we can
store our printed modules. We arranged, sorted and stapled the set of modules given by the
Local Government Unit of Pili (LGU-Pili). We printed additional modules that fulfil the
total number of students for the release of modules on the opening of classes.
2nd Teleconference; Webinar on “Mental Health & Psychosocial Support Services and
Schools Developmental Activities”
This webinar on Mental Health & Psychosocial Support Services and Schools
Developmental Activities tackled the importance of mental health of the teacher on how
they will cope-up with the environment in this new setting as well as how teachers may
help and guide the students regarding their mental health in this time where we are facing
the new normal setting in the modular distance approach.
Continuation of Learning Action Cell (LAC)
Learning Action Cell (LAC) is the teachers’ involvement on how we will give the
students a better understanding of the lessons even we are using the modular distance
learning. We conducted a long week LAC sessions that caters the improvement of the
teachers and how we will improve our knowledge and skill to the better improvement of
teaching in the distance learning.
VII. PROBLEMS ENCOUNTERED
The problems that have been encountered during the said activities are lack of funds and
resources for the additional printing of modules. During the preparation of modules we need a
bond paper of printing, printers, inks and stapler this are the things that usually needed in the
preparation of modules. Also in attending webinars, not every one of us has stable internet
connection and even others encountered poor signal connections that caused delay and
miscommunication to the speakers of the said event.
VIII. RECOMMENDATION
Based on the problems observed it will be more helpful if the school has an additional funds
for printing of modules like solicitations, contributions, and donations or maybe the school can
propose a fund raising activity so that we can provide additional resources for our school to be
used in reproducing modules.

TERMINAL REPORT ON TURN OVER CEREMONY OF PERMANENT SCHOOL
SIITE OF LA PURISIMA HIGH SCHOOL (SJPNHS ANNEX)
I. INTRODUCTION
Year 2020 is the year where we’ve faced a lot of challenges such as COVID 19
pandemic and Typhoons. Despite of those challenges, La Purisima High School is still
blessed to achieve having the permanent school site to continue giving better service to the
students and to our stakeholders. It is an honor and a great privilege for the developing
school to have this.
That is why last October 7. 2020, La Purisima High school in collaboration with
the Department of Education and the Local Government Unit celebrated the TURN OVER
CEREMONY OF PERMANENT SCHOOL SIITE OF LA PURISIMA HIGH SCHOOL
(SJPNHS ANNEX. This is one step forward in preparation for the senior for the school
year 2021-2022. It is a milestone to continue serving quality education among the students,
parents, and stakeholders of La Purisima High School.
II. ACTIVITIES UNDERTAKEN
The first activity is the preparation of the venue and all the materials needed a day
before the said activity. All the functioning committees did their specific functions to make
this event successful.
On the day of October 7, 2020 the event proper it started with the registration and
welcoming of the guests following the health protocols by checking body temperature and
sanitizing. Right then, the program proper started with the invocation through video
presentation and followed by the singing of the Philippine National Anthem led by Nerissa
B. Decepida Teacher I. It was formally opened by giving an opening message by our very
supportive principal Mr. Nelson L. Españo. It was followed by giving inspirational
messages coming from our dearest ASDS Susan S. Collano, CESO VI, PSDS Gilda P.
Bustilla and advocate Barangay Captain of La Purisima, Hon. Sandy C. Bicaldo.
The second part was followed by the vesting of the partial payment amounting to
2.5 million pesos given by municipal mayor Tomas P. Bongalonta to Engineer Gloria B.
Reytana. Followed by the reading the draft of usufruct between the LGU and DepEd. In
MOA signing DepEd Camarines Sur was represented by School Division Superintendent
Loida N. Nidea, CESO V. It is really a great achievement in La Purisima High School that
finally we have permanent school site. Also it is a great partnership that LGU really
supports DepEd to continue in contributing quality education to the learners.
The final part of the activity is the imparting of another inspirational message coming
from our respectful School Division Superintendent Loida N. Nidea, CESO V. and it was
ended by Mr. Sandy A. Saldivar, OIC SJPNHS- La Purisima Annex, who extended his
heartfelt gratitude to those who made this goal turns into reality. By then La Purisima High
School can start to construct building for another level that they may offer for the coming
school year.
III. PROBLEMS ENCOUNTER
Problems are always be part of every activity conducted and encountering this is an
opportunity because it serves as an eye opener if there are still things that can be improve.
During the preparation and on the event proper, we have encountered several problems such
as lack of man power in preparing the tasks and the materials that we are going to utilize in
the activity.
Another problem encountered during the month of October is the typhoon Quinta
where some of the modules of the students were not formally returned to the respective grade
level advisers because some of their house were washed out by the typhoon.
IV. RECOMMENDATION
Based on the problems that we have encountered it will be more helpful if all are
planned a week before the said event and have oriented properly the members of the thing
that they are going to be done on a specific time since there is only a very limited person to
do the task and borrow or buy all the materials needed for the preparation and for the event
proper.
Since typhoon is a natural calamity that we cannot force to stop or to leave our
premises, we give a supplemental learning materials and alternative learning activities.

TERMINAL REPORT FOR THE MONTH OF NOVEMBER 2020
I. INTRODUCTION
We Filipinos experienced a lot of struggles that challenge our lives. We should unite as
one in this new normal to overcome this pandemic that we are facing. The SJPNHS-La Purisima
Annex conducted engagement activities for the new normal education. In this new set up in this
time of pandemic, we the teachers must be equipped with new knowledge and skills to ease the
difficulties in this new normal setting.
II. ACTIVITIES UNDERTAKEN
These following activities are undertaken in the month of November 2020:
Preparation of School
At the very first day of November we experienced a super typhoon named
Typhoon Rolly. We prepared the school to be accommodated by our evacuees. We
arranged and secured the faculty room so that the modules that re intended to distribute on
the next distribution are safe and will not be destroyed by the typhoon.
Preparation of Modules
Right after the calamities we cleaned and arranged the faculty room and rooms
that were affected by the typhoon we made sure that the modules are safe. We arranged,
sorted and stapled the modules that were sent by LGU-Pili and made sure that all our
students will be given modules so we printed additional modules to attain the total numbers
of our students for the next release of modules.
Anduyog Program “Give Love for Typhoon Victims”
Headed by Ma’am Lorenel Interino, a previous Teacher in San Jose Pili
National High School- La Purisima Annex we conducted a simple help through the
Anduyog Program which helped the typhoon victims especially those families who
were totally damaged by the typhoon. The Barangay Captain of La Purisima Hon.
Sandy C. Bicaldo and the school principal Mr. Nelson Españo and Assistant
Principal Ma`am Glenda Liza Tornilla were also in the venue to show their support
in our school.
III. PROBLEMS ENCOUNTERED
After 5 typhoons that we experienced, the problems we have been encountered are: the
school is the evacuation center of the Barangay La Purisima, so it is hard for us to move and
prepare the room that we are using in distributing of modules. Also, the old faculty that the grade
7 teachers used in distributing the modules were damaged by the typhoon. Contacting the parents
in getting the modules is also hard because there is no internet connection and the signal is very
poor because the electric post was damaged. Some of our students were totally damaged by the
typhoon so it is hard for them to get and returned the modules in time.
IV. RECOMMENDATION
Based on the problems observed it will be more helpful if the school has an additional funds
for printing of school materials the school can propose a fund raising activities, contributions or
donations so that we can provide an additional resources for our school especially in reproducing
modules to support the needs of our students..

TERMINAL REPORT FOR THE MONTH OF DECEMBER 2020
I. INTRODUCTION
December has begun. The final month of the year is a time of celebration, and a time that
shimmers with all joyful good wishes we send to one another. Throughout the year, we still face
this continuing health threat and still, we are doing our best to ensure that learning continues and
we adopted the Modular Distance Learning in this new normal to reach all learners to access
quality education regardless of who and where they are.
We, the teachers in San Jose Pili National High School – La Purisima had striven to put our
efforts in distributing and retrieving printed Self-Learning Modules (SLMs) to be utilized by
learners and has a constant communication with their parents. However, one of the major
concerns to ensure that learning continues is the implementation process itself. We need to
consider a lot of factors such as some of our learners that lives on remote areas, parents who are
busy in their works and don’t know how to guide their children in the modules and the
availability of materials to enhance learning. These concerns are not for teachers alone. But also
with the collaboration of help of the schools, stakeholders, and the community to improve the
programs and implement the solutions.
In accordance to Deped annual implementation of In-Service Training, San Jose Pili
National High School – Main held its own school-based seminar together with its annexes to
facilitate professional training and learning opportunities for teacher and discussed the problems
meet and the ways to improve better learning of the new normal in education. It is entitled, IN-
SERVICE TRAINING OF TEACHING AND NON-TEACHING PERSONNEL OF SJPNHS
with the theme, “Restrengthen, Retool and Reboot: Continuing Delivery of Education in the New
Normal” The training covered three days from December 14-16 in the year 2020 at SJPNHS
Pavilion or via virtual.
It is a pertinent will for teachers to continually advance their understanding of the
philosophy and pedagogy of education in order to provide quality and relevant education. INSET
allows teachers to acquire professional goals for the success of the school as a whole and
increase the current level of knowledge and skills in their target areas. It will help mobilize
teachers towards a modernized way of teaching and orient them on the latest trends in the
teaching industry.
“Education is the most powerful weapon which you can use to change the world.” said
Nelson Mandela. Embracing this new normal of learning is where we need to pivot what we
have been doing before to something new to adapt in this type of learning. It gives opportunity
for teachers to gain knowledge on the best way to improve their methods and strategies that is
suited in the present needs in education and in adherence to the growing advancement of
technology in their lives. Culmination of each teacher’s skills, building stronger relationship
between team members, and brainstorming innovative ways towards creating better learning
environment for the learners are some of the ways towards attaining success.
II. ACTIVITIES UNDERTAKEN
Adapting this modular learning and deliver only the most essential learning competencies is
the best way to address the problem to ensure learning continuity and delivery of quality
instruction that are suited to learner’s diversity and the local context. As of now, the modules we
had distributed in all grade levels are up to module 4. The next distribution will be held next
year, 2021. We had contacted and entertained parents and learners if they have queries and
concerns regarding the modules.
This month, we look forward to INSET (In-Service Training) 2020. INSET is conducted
annually to address the continuing professional development of public school teachers. Each
topic covered in the seminar is carefully selected and planned to improve educator’s weak points
and strengthen their strengths.
The INSET was held on San Jose Pili National High School – Main. The week long
INSET 2020 commenced on December 14 – 16, 2020. Registration started at 7:30 am. In the first
day, we attended via virtual. The opening program begun by a prayer led by Jennifer V. Largo
followed by the singing of Philippine National Anthem led by Jasmin G. San Juan. Nelson L.
Españo, SSPIII officially started the program by addressing the crowd in his welcome message.
He then encouraged the teaching and non-teaching personnel of SJPNHS to treat this INSET as
an opportunity to build professional development as well as intensify the camaraderie between
the groups. The participants then enjoyed a Zumba dance to energize the event.
Each speaker took the stage to give a talk on their selected topics that are suited to the
present needs of the new normal in education such as “Meeting the Challenges of the new
Normal in Education” and “Coping Ecological Anxiety.” Before the end of the day, the teachers
were given an opportunity to pose one question that could enhance their understanding of the
topics covered.
The second and third day held on December 15 and 16 at the same venue. Registration

teachers to practice their new knowledge. After a 30-minute break, the School’s Annexes tackled
and reported about the Evaluation and Update of the Learning Delivery Modality and Progress
Report.
The last day of INSET 2020 focuses more on teachers using social media in education. The
third day’s discussions touches on the present needs of education in this new normal that
teachers should adopt innovative ways in teaching. The topics covered for this day are
“Responsible Use of Social Media”, “Re-orientation on Deped Order No. 31, s. 2020” and “Re-
orientation on Division Memorandum No. 265 Protocol on the Use of Internet and Social
Media”. After that, Evaluation and Update of Learning Delivery Modality was tackled by each
Annexes and the progress report by Subject Area. Jael P. Olaño, HTI/GAD Coordinator, gave his

weeklong In-Service Training for 2020. With the support of current School Head, Nelson L.
Españo, the leadership of INSET Focal Person and all the teachers who helped successfully
implemented the said event.
II. PROBLEMS ENCOUNTERED
Problems are indeed part of every activities/trainings conducted and encountering this is a
blessing because it serves as an eye opener if there are still things needed to improve.
During the Module Distribution every week, we have encountered several problems such as
the lack of facilities and equipment like printed SLMs (Instructional materials) and printer.
Therefore, we cannot print and prepare the needed modules because the printer/s sometimes is
malfunctioning due to excessive using of it. Another is, the Schedule of distribution and retrieval
of modules are not followed, where parents are incapable of having gadgets. In that situation, we
are unable to communicate with the parents and learners due to lack of gadgets and sometimes,
parents have work/job to do.
During the 3-day INSET, each topic covered is suitable in the present needs of education.
Since it is held at SJPNHS Pavilion and via virtual, the only problem for virtual is the poor
connectivity. It would be great if it is done virtually, since COVID-19 still spreads but the signal
reception is not good to some areas.
III. RECOMMENDATION
Based on the problems that we have encountered in releasing and retrieving modules, it will
be more helpful if there are a lot of equipment such as printer, bond paper, and the things that are
needed in school. Also, internet connection where signal reception is better, because we used it
as a medium to contact our learners and downloading materials coming from DepEd portals.
