Dear Sir CK,

I am writing to inform you of my decision to resign from my position as OBTI Asst. Manager.
I ll be rendering 30 days notice effective today , for the endorsement and turn over of my tasks. My
last day at work will be on August 3 ,2020.

I am grateful for almost two years (1 year and 11 months) of working with Maxicare and I
have learned a lot and grown professionally during my time of employment. I appreciate the
opportunity to have worked with the company.

I have given this decision a lot of thought , and have decided to move on so I can be able to
pursue my plans to start my own venture. My decision is final and irrevocable and I would greatly
appreciate your consideration.

I will make the best effort to prepare necessary endorsements of assignments and to make
smooth transitions for those who will take my current responsibilities before I leave.
