Please provide the following information related to ………………

1.      Please give a list of all the works related to improvement of roads or streets carried out in my area (please specify your area)
since 1st  April 2000. The list should contain the name of the work, Work Order No, actual date of start, actual date of
completion, amount paid or payable, status of work, name of contractor and mode of improvement of that road or street.

2.      In each of the above works, please give a list of the streets or roads (mentioning from which house no to which house no was it
repaired), which were repaired under that work, the length of repair and average width of repair in each street or road.

3.      For each of these works, please mention what was the guarantee clause, if any and whether it has been invoked so far.

4   After you have prepared the above information, I would like to inspect your measurement books and    

     Work order registers related to these works to ensure that you have given complete information.

I am depositing the application fee (Rs.10/‐) separately/ I am a BPL card Holder, So, I am exempt from payment of fees. My BPL
card no. is:…./ If you feel that above requested information does not pertain to your department then please follow the provisions
of Section 6(3) of the RTI Act, 2005/Also as per the provisions of the RTI Act, 2005 please provide the details (Name and
Designation) of the first appellate authority w.r.t to your department with the reply to the above request., where I may if required
file my first appeal.
