JOINT AFFIDAVIT OF LEGITIMACY

_________________________, after having duly sworn to in accordance with law,
hereby depose and state that:

Annex “A” is attached hereto and made an integral part hereof;

3. At the time of the conception of the said child we were not disqualified by any

4. We are subscribing to this Affidavit for the purpose of informing authorities


concerned regarding the above facts and for the purpose of registering the birth
