AFFIDAVIT

I, Richard T. Condeno, of legal age, Filipino after having duly sworn in


accordance with law, do hereby state:

1. That sometime in January of 2020 I purchased a house and lot in General


Trias City, Cavite.

2. That before transferring to the said property, I caused its repairs, tile
installation, building of kitchen and sala extension, painting, fencing and other
repairs.

3. That the said repair and installation last for more than a month starting from
January 26, 2020 to March 1, 2021.

4. That one of the workers who repaired and installed the tiles is Arvin Gabiola.

5. I often see him every morning before I go to work and around 5:30 to 6:00
PM after my work since I visited the house for checking.

6. That I am executing this affidavit to attest to the fact that Arvin Gabiola was
in Cavite from January 26, 2020 to March 1, 2021.

Affiant further sayeth naught.

AFFIDAVIT OF LOST

I, Erickson I. Infante, Filipino and of legal age after having duly sworn in
accordance with law, do hereby state:

1. That on April 16, 2021, the undersigned went to Ayala Avenue, Makati for an
official business.

2. That when I was about to get my cellular phone to contact the person that I
intend to meet, I noticed that my bag was slashed and my cellular phone was
nowhere to be found therein.

3. That the existing Smart Prepaid Number 09959228032 is very important to


me since my business and personal contacts know that I could be reached
through the said number.

4. That I am executing this Affidavit for purposes of replacing the same number.

Affiant further sayeth naught.

Dear Mr. Camaño,

This refers to your letter dated March 13, 2021 that was actually received by the
undersigned on April 8, 2021. Said letter mandates us to “register with Pag-Ibig
Fund and remit the Pag-Ibig membership savings of our coverable employees”
pursuant to Republic Act 9679.

In response thereto, please be respectfully informed that Carlites Events


Entertainment and Talent Management Center does not have any employee in its
roster since we only hire independent contractors (e.g catering services, lights
and sounds services etc.) everytime we are engaged for an event and those
people working for these independent contractors are not our employees but
employees of the latter. Hence, since there exists no employer-employee
relationship, there is no need for us to pay their Pag-Ibig contributions. Our
company has no regular engagements with companies, entities and private
individuals and only runs on a “per-event basis”. It is also worth mentioning that
our management center has been greatly affected by this global pandemic since
events and gatherings are restricted by the government and some of our clients
opted to cancel their engagement with us.

We thank you for this reminder and rest assured that we have no intention to
deviate from the mandates of Republic Act 9679.
