RETENTION OF RECORDS

1. TITLE: Procedure for the Retention of Records

2. PURPOSE:
To have a documented record on how to retain Laboratory patients record for
future use.

3. SCOPE:
This covers all laboratory records of patients in OPD /In Patients .

4. MATERIALS:

4.1 Ball pen


4.2 Logbook
4.3 Ruler
4.4 Filing cabinet or record rack
5. DEFINITION OF TERMS:

5.1 MTOD – Med Tech on Duty


5.2 Lab. Aide – A Laboratory personnel whose job description is to include but not
limited to record data of patients in the logbook.
5.3 Filing cabinet or Record rack – where all the logbooks of the laboratory is filed and kept.

6. RESPONSIBLE PERSONS:

6.1 Medical Technologist


6.2 Laboratory Aide
6.3 Record officer

8. MONITORING:

8.1 All records of patents in a logbook is properly kept in the Cabinet / record rack.
8.2 Med. Tech./ Lab. Aide to endorse record book to the Record Officer for safe keeping.
8.3 Record office shall in return will acknowledge receipt of such record for future use.
8.4 For future use this records shall be available.

9. DESSIMINATION:

9.1 To inform all Laboratory staff of the procedure on how to retrieve records of lab.
Examinations of each patient for needs arises.
9.2 To be informed also that records have number of years to be kept in a record room and
is discarded after a certain number of years of retention. This will follow the Standards of
retention of Records promulgated by the DOH and other Agency.
