Diagnostic Grouping: Definition: A group of conditions due to an impairment in
Developmental Disability physical, learning, language or behavior areas.

Specific Diagnosis: Down Syndrome Definition: A condition in which a person has an extra
Trisomy 21. chromosome.

Qualifying Symptoms:
Trisomy 21: 95% of people with Down syndrome. Each cell in
their body has three separate copies of chromosome 21 instead
of the typical two copies.
Translocation Down syndrome: 3% of people with Down
syndrome. A whole extra or an extra part of a chromosome 21
is present but is “trans-located” or attached to a different
chromosome.
Mosaic Down syndrome: 2% of people with Down syndrome.
People with Mosaic Down syndrome have a combination of
cells that have three copies of chromosome 21 but other cells
still have the typical two chromosomes. Typically have fewer
features of the condition.
Identified Problems:
Flattened face (especially the bridge of the nose).
Almond- shaped eyes that slant upwards.
Shorter than average neck.
Small ears.
Tongues typically sticks out the mouth.
Small hands and feet.
A single line across the palm of the hand (palmar crease).
