Dear Sir / Madam, I, Mr./Mrs. ____________________________, parent of

______________________, who is a student of Class _______ in Delhi Public School BN, voluntarily
give consent to my ward to attend PREBOARD 2 from 18 th October 2021. I also declare that my ward
does not exhibit any symptoms of COVID 19 and he / she will follow all safety and hygiene guidelines
given from school. I shall also make sure that I do not send my ward to School if she/he exhibits any
symptoms of ill health. I will not hold the school accountable in the event of any unforeseen incident. I
shall arrange to pick my ward in case he / she exhibits any symptom at School, as soon as I receive any
such intimation from the school.
