WARD
TRANSDERMAL PATCH.
Good day! A non-invasive and painless method of drug delivery will be demonstrated in today’s video. With
the added benefit of providing a constant and consistent therapeutic dosage over a predetermined time
period, a medication known as a transdermal patch. To properly do this:
a. Bring equipment to patient’s bedside. Check the medication order against physician’s order according to
agency policy to save time.
b. Identify the person by checking the identification band on the patient’s wrist and asking the patient his or
her name to prevent error.
c. Ask patient about any allergies.
d. Explain the procedure to the patient.
e. Perform hand hygiene and don gloves.
f. Assess the patient’s skin where patch is to be placed, looking for any signs of irritation or any areas that
are broken down. Find place that does not have a large amount of hair on chest or back, scissors may be
used to trim hair. Do not shave hair.
g. Remove any old transdermal patches that are left on patient. Gently wash the area where the old patch
was, with soap and water.
h. Remove cover without touching the adhesive side. Lie patch on patient’s skin, pressing firmly with palm of
hand for 10 seconds.
i. Document patient’ reaction to the medication, patient’s comfort and other significant findings.
STEAM INHALATION.
Good day! Many people, especially in our current situation, are at risk of developing respiratory difficulties.
Aside from drugs, there is another therapeutic option, which is the steam inhalation therapy. It entails inhaling
water vapor that can relief common cold, as well future respiratory problems. To properly do this:
a) You would need a bowl of steamed water. Carefully pour the hot water into the bowl on a table. You can
also add some ingredients like salt, leaves or eucalyptus oil.
b) After the bowl is put together, you will inhale the steam. Drape the towel over the back of your head to
prevent the steam from escaping.
c) Turn on a timer.
d) Shut your eyes and slowly lower your head toward the hot water until you’re about 8 to 12 inches away
from the water.
e) Inhale slowly and deeply through your nose for at least two to five minutes. Avoid doing the session for 10
minutes. You can repeat the session twice a day.
f) Once done, remove the towel and wipe your face with tissue or a clean cloth.
OPERATING ROOM
SURGICAL SCRUBBING.
Good day! Surgical site infections remain a prevalent threat to patient safety. One of the most significant
procedures necessary to limit the threat or danger of microorganism contamination during surgical
procedures is to perform the surgical scrubbing which I will be demonstrating in today’s video. To properly do
this:
a. Before beginning the procedure, you must remove all jewelry from both hands and arms. Inspect your
skin and nails. Ensure your skin is free of any cuts wounds. Nails must not extend past your fingertips.
Remember, acrylic and artificial nails, and nails with foreign bodies are not allowed in the operating room.
Put on your personal protective equipment. Make sure your hair and ears are tucked into your surgical
head covering. Don proper eye protection and a surgical mask your nose, mouth and if applicable your
facial hair.
b. Take a scrubbing brush. Inside the container, there is a brush on one side, sponge on the other and a nail
pick.
c. Open the packet and leave the brush resting its container by the sink.
d. Washing of hands and forearms for 1 minute. Using your hands, turn on the taps. Rinse your hands and
forearms with warm water then use scrub solution and thoroughly wash from finger tips to elbows for 1
minute. Always keep your elbows elevated to avoid re-contamination of the hands once they have been
washed.
e. Cleaning of nails. Firstly, lather your hands and forearms with scrubbing solution then take the nail pick
and clean each nail. Rinse nail every after picking. Rinse hands and forearms. Now, its time to pick up the
brush and apply scrub solution and water to the brush while scrubbing your nails on both hands. Lastly,
rinse your hands and forearms.
f. Washing of hands and forearms again for 3 minutes. This time wash your hands to your elbow using the
scrub solution and then rinse off. Pay particular attention to cleaning your hands in this step.
g. Turning off the taps. Do this with your elbows and remember to keep your hands elevated.
h. Drying off arms. Dry each arm and turn using a different towel for each arm. The skin should be blotted
dry starting at the fingertips down to the elbows. Discard towel after used.
ABDOMINAL PREP.
Good day! A critical step in reducing the risk of a surgical site infection for patients undergoing surgery which
is called the surgical skin/abdominal prep will be demonstrated in today’s video. To do this properly:
a) Do a hand washing first.
b) Now, open the prep (3 towels, cotton balls, pick, 2 solutions).
c) And we are now going to open glove.
d) Put a protective sheet underneath the patient on both sides.
e) To proceed, pour the first solution which is the scrub solution in a compartment, then soak sheets. Next to
pour is the paint solution. Note that the first solution is for cleansing and the other one is for painting.
f) To start cleansing, soak the tip on a scrub solution and start cleansing at the umbilicus. And why is that? It
is because it is the dirtiest part of the abdomen, we don’t want to start at the cleanest down to the
dirtiest, what’s the point of the prep right? Now there’s gonna be rules and different things we are going
to do. We are going to clean in an up and out direction.
g) The main reason we want to make sure when doing this is to stay in the incision line. Once you go beyond,
you don’t come back to the incision. Always remember to stand on the same side of your incision, don’t
stand opposite of the incision line.
h) Now, there’s a couple of different ways to do an abdominal prep and we will be using this method. Make
an up and down motion and make a u motion and each time you make this motion you are coming higher
reaching the nipple line.
i) Note that after applying the scrub solution, there will be a lot of bubbles and so you should take a drying
towel. Put it on your patient and blot the soap away. When lifting up the towel, make sure that this one
comes up so when you lift it up its not dragging. And disregard the towel in a garbage.
j) Now, we are going to paint. Remember that paint is different from scrub by putting it once. To do this,
you go down once and come back the other side and do the u motion and each you do this you go further
and further reaching again the incision line. Lastly, apply the paint around the symphysis pubis to make all
the abdomen part clean.
k) After painting, wait for the solution to set on the skin of the patient for at least 3 minutes and when the
solution is set off, take off the two protective sheets underneath the patients both sides.
