Madam:

Christian greetings!

This is to request transfer from my present station Dolores Integrated School, Quirino
District, Quirino, Isabela to Mabini National High School, Mabini, Gamu, Isabela, due to the
following reasons:

1. I am a breastfeeding mom of a 4 month-old baby who is struggling to adjust to bottle feeding.


2. I am serving for 8 years and 7 months in my present station.
3. Going to my present station is a challenge for me especially when it rains hard because the
distance of our simple abode is too far from my present station.
4. Less expenses from the diesel to my daily meal.
5. My mother needs a medical attention and care due to her fluctuating blood pressure.
6. My father and my mother who is responsible to my children whenever I go to school is
already a senior citizen. It would be better if I will assign in a school near to our abode so that
it is easier for me to respond to their call especially when it is emergency.
7. My children need someone who can guide them in their daily needs.

This is to request transfer from my present station Dolores Integrated School, Quirino
District, Quirino, Isabela to San Juan Elementary School, San Juan, Quirino, Isabela, due to the
following reasons:

1. I served 3 years and 3 month in my present station.


2. Going to my present station is a challenge for me especially when it rains hard because the
distance ( 9 km )of our simple abode is too far from my present station.
3. Less expenses for the gasoline and my daily meal.
4. My mother who is under my care needs a medical attention and care due to her fluctuating
blood pressure and severe asthma.
5. I want to serve in my barangay because I believe I will be more productive and effective
teacher if my station is near my residence.
6. There is a vacancy in the said school due to retirement.
It would be a great favor if you consider my request regarding this matter.
