INTRODUCTION
BLOOD BANK:
Blood bank means, a centre within an organization or an
institution for collection, grouping, cross-matching,
storage, processing and distribution of Whole Human
Blood or Human Blood Products from selected human
donors.

INTRODUCTION:
A well organised Blood Transfusion Service (BTS)
is a vital component of any health care delivery
system. An integrated strategy for Blood Safety is
required for elimination of transfusion transmitted
infections and for provision of safe and adequate
blood transfusion services to the people. The main
component of an integrated strategy include
collection of blood only from voluntary, non-
remunerated blood donors, screening for all
transfusion transmitted infections and reduction of
unnecessary transfusion. The standards vary from
State to State, cities to cities and centre to centre in
the same city. In spite of hospital based system,
many large hospitals and nursing homes do not have
their own blood banks and this has led to
proliferation of stand-alone private blood banks. The
blood component production/availability and
utilization is extremely limited. Thus, a need for
modification and change in the blood transfusion
service has necessitated formulation of a National
Blood Policy and development of a National Blood
Programme which will also ensure implementation
of the directives of Supreme Court of India - 1996.

MISSION:
Policy aims to ensure easily accessible and adequate
supply of safe and quality blood and blood components
collected / procured from a voluntary non-remunerated
regular blood donor in well equipped premises, which is
free from transfusion transmitted infections, and is stored
and transported under optimum conditions.

OBJECTIVES OF THE POLICY:


- Commitment to provide safe and adequate
quantity of blood, blood components and blood
products.
- To make available adequate resources to develop
and re-organize the blood transfusion services in
the entire country.
- To make latest technology available for operating
the blood transfusion services and ensure it’s
functioning in an updated manner.
- To launch extensive awareness programmes for
donor information, education, motivation,
recruitment and retention in order to ensure
adequate availability of safe blood.
- To encourage appropriate clinical use of blood
and blood products.
- To strengthen the manpower through human
resource development.
- To encourage Research & Development in the
field of Transfusion Medicine and related
technology.
- To take adequate regulatory and legislative steps
for monitoring and evaluation of blood
transfusion services and to take steps to eliminate
profiteering in blood banks.
Guidelines for Opening and Licensing
blood bank
Application for grant or renewal of
license:
• Application for grant or renewal of license for
operation of blood bank shall be made to the
Licensing Authority in Form 27-C and shall be
accompanied by license fee of Rs. Six hundred
(Rs.600/-) and inspection fee of Rs. Two
hundred (RS. 200/-) in the case of renewal of
license. if the Applicant applies for renewal of
license after its expiry but within six months of
such expiry, the fee payable for the renewal of
the license shall be Rs.600/- plus an additional
fee at the rate of Rs.200 per month or a part
thereof in addition to the inspection fee.
• A fee of Rs.100/- shall be paid for a duplicate
copy of a license issued, if the original is
defaced, damaged or lost.
Pre-requisite for grant of license for
Blood Bank(Rule 122G)
• The operation of the Blood Bank shall be carried
out under the active direction and personal
supervision of competent technical staff
consisting of at least one person who is whole
time employee, a Medical Officer who is a
Graduate in Medicine of a University recognized
by the Central Government having experience in
Blood Bank for 6 months during regular service.
Applicant shall provide adequate space, plant
and equipment for any or all the operations of
blood collection or blood processing. Applicant
shall provide and maintain adequate technical
staff. Applicant shall provide adequate
arrangements for storage of Whole Human
Blood, Human Blood components and blood
products. Applicant shall provide and maintain
adequate technical staff.

Inspection:
• Blood Bank is proposed to be inspected by one or
more inspectors, appointed under the Act and/or
along with the expert in the concerned field. The
Inspector or Inspectors shall examine all portions
of the premises and appliances/equipment and
also enquire into the professional qualification of
the expert staff and other technical staff to be
employed. If within a period of six months from
the rejection of application for a license, the
Applicant informs the licensing authority that the
conditions laid down have been satisfied and
deposits an inspection fee of RS50. Any person
who is aggrieved by the order passed by the
licensing authority or central license approving
authority, as the case may be, may within 30 days
from the date of receipt of such order, appeal to
the State Govt. or Central Govt., as the case may
be, after such enquiry into the matter.
Duration of license:
• An original license in Form 28-C or a renewed
license in Form 26-G, unless suspended or
cancelled shall be valid up to the 31st December
of the year.
• Cancelation of license: cancel a license issued
under this part or suspend it for such period as
he thinks fit, either wholly or in respect of some
of the substances to which it relates, if in his
opinion, the licensee has failed to comply with
any of the conditions of the license or with any
provision of the Act or Rules there under. A
licensee whose license has been suspended or
cancelled may, within 3 months of the date of
the order.
Condition: special conditions set out in
Schedule F, Part XII-B and Part XII-C…….
The licensee shall provide and maintain
adequate staff, plan and premises for the proper
operation of a Blood Bank for processing whole
human blood licensee shall maintain staff,
premises and equipment as specified in Rule
122-G. The licensee shall maintain necessary
records and registers as specified in Schedule F,
Part XII-B and XII-C. The records and registers
shall be maintained for a period of five years
from the date of manufacture. Licensee shall
maintained/preserve reference sample and
supply to the Inspector the reference sample of
the whole human blood collected by him in an
adequate quantity to conduct all the prescribed
tests. The licensee shall supply to the Inspector
the reference sample for the purpose of testing.
Licensee shall allow an Inspector appointed
under the Act to inspect all registers and records
maintained under these rules.
Equipment:
• For blood collection room,
• Donor beds or tables: It shall be suitably and
comfortably cushioned and shall be of
appropriate size.
• Bed side tables.
• Sphygmomanometer and Stethoscope.
• Recovery beds for donors.
• Refrigerators: Maintaining temperature between
4 to 6 degrees C with recording thermometer
and alarm device, weighing machine.
Hemoglobin determination:
• Copper sulphate solution (specific gravity
1.053).
• Sterile lancet.
• Capillary tubing (1.3 to 1.4 x 65 mm or Pasteur
pipettes).
• Rubber bulbs for capillary tubing.
• Sahli’s haemoglobinometer/calorimetric
method.
Temperature and pulse determination:
• Clinical thermometers.
• Equipment and materials for aseptic cleaning
of the thermometer and Watch (with a second-
hand needle).
Blood containers:
• Disposable plastic packs (closed system) as per
the specification of USP.
• Blood collection bottles: 540 ml. with graduated
capacity of up to 500 ml graduation mark
• Anti-coagulants: Anti-coagulant solution shall
be sterile, pyrogen free.
• Citrate phosphate dextrose solution (CPD) shall
be required for 100 ml of blood.
• Acid Citrate Dextrose Solution (A.C.D. &
Formula-A) IP Grade 15 ml. Solution shall be
required for 100 ml. of blood.
Personnel:
• Doctor- Degree in Medicine of a University
recognized by the Central Government having
experience in Blood Bank for 6 months during
regular services.
• Registered nurse.
• Blood Bank technician with MLT qualification
or its equivalent having adequate experience in
blood grouping and serology work.\
• Laboratory Assistant with MLT qualifications
or its equivalent.
• Laboratory Attendant.
Records & Labels:
• Blood Donor Register – Indicating serial
number, date of bleeding, name of donor with
particulars, age, weight, hemoglobin, blood
pressure, medical examination, signature of
Medical Officer bleeding the donor, bottle bag
number.
• Blood Stock Register – Indicating bottle bag
number, date of collection, date of expiry,
quantity in ml., ABO/Rh Group, results for
testing of HIV antibodies, malaria, VDRL,
Hepatitis-B surface antigen.
• Issue Register – Indicating serial number, date
and time of issue, bottle number, ABO/Rh
group, total quantity in ml., name of the
recipient, group of recipient, unit/institution,
particulars of product supplied (whole human
blood, red cell/platelet, concentrates,
cryoprecipitate etc), quantity supplied,
compatibility report, signature of issuing
persons.
• Register for ACD/CPD/CPD-A – Bottles/packs
giving details of firm, batch number, and date of
supply and results of testing.
• Blood Bank must issue the cross matching report
of the blood of the patient along with the blood
bottle.
• Transfusion Adverse Reaction Records( fever,
shortness of breath, red urine)
• Records of Purchase, use and stock in hand of
disposable needles, syringes, plastic bags, sets
shall be maintained.
Labels:
• The serial number of the bottle.
• Date on which the blood is drawn and the date
of expiry as prescribed under Schedule P
• The results of the tests for Hepatitis, Syphilis,
freedom from HIV antibodies.
• Name and address of Blood Bank.
• License number.
• Instruction to keep continuously at 4-degree to 6
degree C.
