Affidavit of Loss of Identification Card

Jeff Renz C. Balbuena filed an affidavit stating that he lost his Military ID card number 877810 issued by the Armed Forces of the Philippines. AFFIDAVIT OF LOSS OF IDENTIFICATION CARD

I, JEFF RENZ C. BALBUENA, Filipino, of legal age, married to Lillian F.


Balbuena and a resident of Pawili, Bula, Camarines, after being sworn in
accordance with law, hereby depose and say:

1. That I am the holder of a Military Id Card number 877810 issued by


the Armed Forces of the Philippines.
2. That I used to secure and put it into my wallet, among others;
3. That sometime on October 22, 2020, when I arrived home from our
duty, I discovered that my Military Id was no longer in my possession.
4. That after diligent effort to locate the same had proved futile;
5. That said Military ID was not confiscated by the authorities of the
Armed forces of the Philippines by reason of any violation of their rules
and regulations;
6. That I am executing this affidavit to attest the truth of the foregoing
facts and to request for a replacement of said Military ID.
