AFFIDAVIT OF UNDERTAKING

______________________, after having been sworn to in accordance with law, do hereby depose and state that:

1. I am fully aware of the provisions of the following laws, their implementing rules and regulations, as well as all
the DENR Administrative Orders, and other pertinent rules and regulations, to wit:

a. Presidential Decree No. 1586 (Establishing and Environmental Impact Statement System
Including Other Environmental Management Related Measures and for other Purposes);

b. Republic Act No. 6969 (Toxic Substances and Hazardous and Nuclear Wastes Control Act of
1990);

c. Republic Act No. 8749 (Philippine Clean Air Act of 1999);

d. Republic Act No. 9003 (Ecological Solid Waste Management Act of 2000); and

e. Republic Act No. 9275 (Philippine Clean Water Act of 2004);

2. I shall ensure the proper performance, operation, and maintenance of environmental management facilities or
systems of the establishment;

3. I shall promptly submit duly notarized and accomplished Compliance Monitoring Reports (CMR) and Self
Monitoring Reports (SMR);

4. I shall immediately notify the Environmental Management Bureau Region XI (EMB-XI) of any environmental
incident involving our establishment;

5. I shall facilitate the compliance of the establishment of the requirements that may from time to time be
prescribed by the Department and recommend to the management the installation and operation of additional
equipment for the abatement of pollution;

6. I likewise undertake to be SEVERALLY LIABLE for any violation of PD 1586, RA 6969, RA 8749, RA 9003,
RA 9275, their respective IRRs, and other pertinent rules and regulations committed by our establishment;

7. I am executing this Affidavit of Undertaking in order to attest to the truth of the foregoing facts, to inform the
Environmental Management Bureau Region XI, and other government agencies, and for whatever legal
purpose it may serve.
