120/min Temperature- 38.7 O2 Saturation- 89%

asthmatic attack. He has recovered quickly and is stable now. He is getting discharged
today and was prescribed with three medications which are as follows:

Salbutamol PRN , Beclomethasone 400 microgram BD, Prednisolone 30 mg OD 3


days.

complaints of headache.

Who the patient is: Olivia Jackson 63 year old was brought to the hospital by her
husband due to weakness of one side of the body, slurred speech, drooping of the
angle of mouth and difficulty swallowing.

Other Information: Olivia’s condition improved after 2 hours and she is completely fine
now. Blood pressure was measured and was recorded as 150/100 mm of Hg. Routine
blood tests were done and the results are being awaited for the same. General and
Neurological examination was done and there is no findings present

Special Note: Consent has been taken from Olivia Jackson to talk to her husband
Michael Jackson.

What you must do: Talk to the patient’s relative and take focused history and discuss
appropriate further management with him.

She has not passed stool since she was admitted.

Other Information: She was given Co-codamol for her pain which is controlled now.
She is now stable.

Who the patient is: Marina Myers 62 year old was admitted in the ward with a pelvic
fracture and was diagnosed with Osteoporosis. She developed urosepsis while she was
under treatment which was managed with antibiotics. She is mot being discharged and
wants to know about the medications that have been prescribed to her which are as
follows:
1. Amoxiclav 500 mg TDS 5 days
2. Codeine 1 tab PRN
3. Alendronate 70 mg mane
4. Calcitriol BD
5. Lisinopril 5 mg OD
6. PCM 2 tabs BD
7. Atorvastatin 10 mg OD
8. Laxido sachet PRN

Other Information: She was previously on Lisinopril 10 mg which has been changed to
5mg due to dizziness.

Who the patient is: Ashley Adams 65 year old female came to the clinic for
assessment.

Other Information: Patient has been diagnosed with Rheumatoid Arthritis. Patient is on
the following medication: Methotrexate PO 7.5 mg per week, Paracetamol PO up to 8
tablets, Folic Acid PO. Her arthritis is under control and blood levels for methotrexate is
normal.

HIV antibody and p24 antigen test are positive.
