Task 1 The reasons for managing safely
To avoid the additional expenses on the incidents which were not formally reported, and to act
into the legal notices from local bodies urging Infinity Warehouse Pte. Ltd., to take action to
prevent injuries, the moral, legal and financial reasons for managing safety are hereby
presented:
A. MORAL & LEGAL REASONS OF MANAGING SAFETY.
THAT THE ORGANIZATION OR MANAGEMENT HAS THE DUTY OF CARE TO THE
WORKERS AS MANDATED BY LAW, ILO-OSH 2001, 3.3, RESPONSIBILITIES AND
ACCOUNTABILITIES PROVIDES THAT THE ORGANIZATION SHOULD:
1) Establish and implement effective arrangements to identify and eliminate or control
work related hazards, whilst;
The management identified that there was an increase in incidents in the
warehouses.
2) Define and communicate to the members of the organization, whilst;
The site head keeps on interrupting and tries to minimize the communication
while the safety officer tied to ask questions with the workers.
The site head shouted at the worker while having food at the loading bay
resulting the worker to stop eating.
3) Provide proper training to ensure that all persons are competent to carry out the
safety and health aspects while performing their duties and responsibilities, whilst;
The wrapping machine operators were operating the machine at a speed higher
than that recommended in the machines chart. The operator only got hands on
instruction by the previous operator for half an hour who had resigned due to
health issues.
4) Provide safe working environment, whilst;
The path ways are obstructed with material and there is poor housekeeping in
most areas including the office area where files and office supplies are placed in
a disorderly way. The level of lighting is low and even the ventilation is poor.
5) Provide proper venue for dinning with adequate drinking water supply, and some
other venue for personal necessities like smoking shelter, Whilst;
One of the workers was having food at the loading bay.
There are no arrangements for drinking water and workers have to walk a fair
distance to the pantry area to refill their bottles.
The truck drivers were smoking near the security gate.
6) Provide room and Lockers for the worker’s extra clothes’ whilst;
The workers keep their bags near the work location. This is because there is no
change or locker rooms provided.

or first aid injuries as most feel it is unnecessary and a waste of time. Also they
did not know that such a formal system exists.
10) Provide proper venue for the movement of people and vehicles, Whilst;
There is no fixed place for parking and they park their vehicles wherever there is
space.
B. FINANCIAL REASONS OF MANAGING SAFETY.
THAT THE ORGANIZATION OR MANAGEMENT HAS THE DUTY TO PROTECT THE
ORGANIZATION AGAINST FINANCIAL LOSS, whilst;
The main objective behind the recruitment of the safety officers to avoid the
additional expenses on the incidents which were not formally reported. Also the
organization has received legal notices from local bodies urging them to take
action to prevent injuries.
THE ORGANIZATION MAY SUFFER FINANCIAL LOST BECAUSE OF THE FOLLOWING;
1) The issuance of the legal notice would there must be SAFETY CULTURE AUDIT with
external auditors, the presence of external auditors involve a lot of financial cost.
2) There are hand and leg injuries caused due to manual handling.
3) FLT badly damaged and needs costly replacement spares.
4) Due to the intervention of local authorities, the organization might pay a lifetime
pension to the injured workers if they suffered a permanent damage.
5) Because of the incidents the organization suffers the downfall of sales due LTI.
Task 2 Roles and Responsibilities
Employers are duty bound to comply with Duties and Responsibilities as prescribe the ILO-OSH
2001 3.3, but in the case of Infinity Warehouse Pte. Ltd., the management FAILED to perform
the duties, such as:
1) Providing proper venue for dinning with adequate drinking water supply, and some
other venue for personal necessities like smoking shelter, Whilst;
One of the workers was having food at the loading bay.
There are no arrangements for drinking water and workers have to walk a fair
distance to the pantry area to refill their bottles.
2) Providing room and Lockers for the worker’s extra clothes’ whilst;
The workers keep their bags near the work location. This is because there is no
change or locker rooms provided.
3) Provide proper and well maintained first aide material with a licensed or well-trained
first aider, whilst;
Many workers shared their thoughts regarding not having proper first aid

The forklift was parked near to the fire hydrant blocking it. On inquiring the
driver said that there is no fixed place for parking and they park it wherever
there is space.
Task 3 Contractor management
(a).
Training should be conducted before works for fire system be allowed to start working, they as
follows:
1) Fire safety
2) How to eliminate fire
3) How to use fire extinguisher
4) First aide
5) Evacuation technique or procedure.
(b).
The contractors should give the assurance that they can perform their activities without any
incident by:
1) Providing the organization a documented risk assessment.
2) Provide a written set of procedures on how they will carry their task.
3) Provide supervisors in proportion to the number of workers.
4) Provide safety officers in proportion to the number of workers.
5) Providing daily checklist on their firefighting equipment’s.
Task 4 Health and safety culture
There is a lot of negative Culture practice in the Infinity Warehouse Pte. Ltd., they are:
1) Site manager doesn’t allow the safety Officer to speak to any of the worker.
2) When the safety officer asked questions to workers, site head keeps on interrupting and
tries to minimize the communication with the workers.
3) At the loading bay one of the workers was having food, site head shouted at him
resulting the worker to stop eating.
4) There are no arrangements for drinking water and workers have to walk a fair distance
to the pantry area to refill their bottles.
5) The workers keep their bags near the work location. This is because there is no change
or locker rooms provided.
6) The path ways are obstructed with material.
7) There is poor housekeeping in most areas including the office area where files and
office supplies are placed in a disorderly way.

9) Loaders were unloading material manually and carrying the load to the respective
locations by hand as the only fork lift truck (FLT) was under maintenance.
10) The forklift was parked near to the fire hydrant blocking it. There is no fixed place for

11) The condition of the forklift was not good and it was badly damaged.
12) Workers were working with a ladder which was not supported.
13) Majority of extinguishers are due for refill.
14) The truck drivers were smoking near the security gate.
15) No body reports near misses or first aid injuries as most feel it is unnecessary and a
waste of time.
Task 5 Incident management
(a).
Infinity Warehouse Pte. Ltd., Culture shows a lot of unsafe acts or behavior, 3 of which are:
1) The wrapping machine operators were operating the machine at a speed higher than
that recommended in the machines chart.
2) Workers were working with a ladder which was not supported.
3) There is no fixed place for parking and they park their vehicles wherever there is space.
(b).
Most of the workers are often times force to do some unsafe acts or behavior due to the failure
manage to show their to commitment to good and safe safety culture. In the case of Infinity
Warehouse Pte. Ltd., the are as follows:
1) Site manager doesn’t allow the safety Officer to speak to any of the worker. When the
safety officer asked questions to workers, site head keeps on interrupting and tries to
minimize the communication with the workers. This situation limits the capability of the
safety officer assist and determine the precautions to be done to eliminate hazards and
introduce safety culture to the work place.
2) No training was being conducted for all workers in last six months. Such failure of the
management resulted to the wrapping machine operators were operating the machine
at a speed higher than that recommended in the machines chart, such action of the
untrained operator may not only to incident or accident in the part of the operator but
also to the community as a whole.
3) At the loading bay one of the workers was having food, site head shouted at him
resulting the worker to stop eating. For the fact that a worker was eating at the loading
bay, this only shows that there is a failure in the side of the management to provide
proper dining area, causing the worker to eat in the loading or it is in some other
available site spaces. This may result also into improper food waste disposal and could
attract some insect, flies; dogs and event snakes migrate to the worksite. The presence
of these insects and unwanted animals is hazardous not only the workers but to the
community as a whole.
4) There is low level of lighting and even the ventilation is poor. Low level of lighting may
cause the worker to suffer eye-strain, fatigue, headaches, stress and even accidents.
On the other hand, poor ventilation may also cause the building up of carbon dioxide
and lessen the oxygen level that could lead the workers suffering from shortness of
breath, headaches and fatigue.
5) The path ways are obstructed with material.
6) There is poor housekeeping in most areas including the office area where files and
office supplies are placed in a disorderly way.
7) There is no fixed place for parking and they park their vehicles wherever there is space.
Task 6 Incident reporting and investigation
(a).
The incident where the FLT toppled sideways while taking a turn at a high speed. The incident
must be investigated to find out the main cause and the intervening for the incident.
The investigation must cover the following:
1) Is the FLT certified for the safe use by the third party? Is there a valid inspection
sticker attach on the FLT to attest that it has been inspected and is safe to be used?
2) Is the operator certified or licensed to operate the said FLT.
3) Is there an updated daily inspection check list filled up by the operator before staring
the daily operation?
4) Is there a flagman assigned to assist the operator during movement of the FLT?
5) Is there a standard traffic sign boards that that tells about trajectory of the road and
the allowable speed limit.
6) Is the road well lighted to allow 360 degrees visibility?
7) Is the road free of obstruction to allow the operator to safe maneuver the FLT, there
must be an updated daily inspection check list filled up by the operator before staring
the daily operation
(b).
To prevent he incident where the FLT toppled sideways while taking a turn at a high speed to
happen again in the future. The following safety measures must be IMOLEMENTED:
1) Ensure that the FLT is certified for the safe use by the third party, there must be a valid
inspection sticker attach on the FLT to attest that it has been inspected and is safe to
be used.
2) Ensure that there must be a standard traffic sign boards that tell about the direction of
the road and the maximum allowable speed limit.
3) Ensure that there must be a flagman to assist operator during the movement of the
FLT, and to assure that the travel path is free from any obstruction
Task 7 monitoring and measurements
IN ORDER TO REDUCE THE INCIDENTS, ACTIVE MONITORING SHOULD BE IN PLACE. SUCH
AS:
1) Ensure that the toolbox talk should be conducted before starting the work.
2) Ensure that the workers are having food at the designated dining area.
3) Ensure that there is an accessible drinking water.
4) Ensure that the practice having a proper housekeeping.
5) Ensure that there is lighting and even the ventilation.
6) Ensure the availability dressing and locker rooms.
7) Ensure that the FLT is park in the designated area.
8) Ensure the proper use of ladder.
9) Ensure operators operate the machine at a speed limit recommended in the machine
chart
10) Ensure the availability of proper first aid kit or PPE.
Task 8 Emergency and First Aid
Considering the nature of work and activities, the FIRST AID should be prepared in
consideration of the following factors:
1) 1,000 people including costumers.
2) Hazardous chemicals.
3) Injuries resulting from manual handling, slips, trips and fall due to poor housekeeping,
improper use of ladder.
4) Minor or serious injuries resulting from traffic accidents.
5) Lungs problem due to poor ventilation.
6) Eyes problem due to low lighting.
7) Dehydration issues resulting from inadequate water supply.
8) Blood pressure problem especially for night workers.
9) Stress and fatigue as the organization very customer driven and offer quick service and
after sales.
10) Transportation in case of serious physical injury as The FLT toppled sideways while
taking a turn at a high speed.
Task 9 Training
(a).
TRAINING PROVIDED IN THE WAREHOUSE IS OBSERVED TO VERY LOW.
A) The organization showed a very a negative culture, that resulted into a significant
incidents and accidents, it is revealed that the level of TRAINING provided in the
warehouse is very LOW. Many workers shared that there was NO training being
conducted for them in last six months, whilst;
1) Workers were working with a ladder which was not supported, which is obviously
may lead into a serious incidents or accidents.
2) The wrapping machine operators were operating the machine at a speed higher than
that recommended in the machines chart. The operator replied that he never knew
that the speed was high and he had been always operating it at the same speed
from the day he had joined. On inquiring, whether he had received any formal
training in operating the machine, the operator replied that he got hands on
instruction by the previous operator for half an hour.
3) Workers reported that the only FLT they have is under maintenance due to an
accident that happened. The FLT toppled sideways while taking a turn at a high
speed. The driver was lucky enough to escape uninjured but the impact left the FLT
badly damaged and needs costly replacement spare.
(b).
B) Considering the above observations and the rest of the incidents accidents happening in
site, the following training are hereby strongly recommended:
1) Training on Safe Use of the Ladder.
2) Training on Work at Height.
3) Training on Manual Handling.
4) Training on Using and operating the wrapping machine.
5) Training on Safety Driving.
6) Training for Flagman to assist the movement of the FLT.
7) Training on Fire Prevention.
8) Training on First Aide.
9) Training on Proper Housekeeping.
Task 10 Workers consultation
(a).
The management has hired you as a corporate HSE officer. The main objective behind
your recruitment is to avoid the additional expenses on the incidents which were not formally
reported. Also the organization has received legal notices from local bodies urging them to
take action to prevent injuries.
In order ovoid an additional expenses on accidents and or accident and to take actions on
the legal notice issued by the local bodies to prevent injuries, a CONSULTATION with the
workers must be in place, such as, the following:
1) Daily Toolbox Talk must be conducted before the activity start, stating the previous
incidents, near misses or accidents in the in the past or previous, and solicit from the
works on how they avoid such to happen again before the safe officer would present his
ways on avoiding the hazards.
2) Weekly General Safety Meeting must be conducted to ensure that the general safety of
the entire working area especially on the people and vehicle movement within the area,
whilst, to avoid the accident involving the FLT.
3) Weekly walk through by the management on the site. Findings and or observations
must be communication to the workers for a possible action for improvements.
4) Provide the FLT operation a check list of the FLT to be filled in before operation will take
place, in a way the operator could make the management aware on the condition of the
FLT.
5) Discuss with the wrapping machine operator the operator’s manual, then, ask him
comment on how he is going to safely use the wrapping machine.
6) Assigned supervisor in every area who is responsible to communicate directly to the
worker and is responsible to carry the workers concern to the management.
7) Regular training on health and safety issues must be provided a monthly basis, during
the said training, the workers should be given a chance to ear out their issues and
concern.
(b).
Provided that the above way on consulting the general safety of the organization’s operation
be put into practice, chances are:
1) FINANCIAL FLOW OF THE ORGANIZATION WILL BE PROTECED.
It may be direct or indirect cost.
Direct cost. Cost for the hospitalization of the workers suffering hand and
injury after manual loading without being trained on safe manual handling.
Direct cost. Costly replacement spare parts of the FLT after being damage
resulting from overturning.
Direct cost. Upon the intervention of the local legal bodies, the organization
might be obligated to pay sick leave for the injured workers while recovering at
home, or it might be pension damage of injury is permanent.
Indirect cost. Upon the legal notice coming from the local legal bodies, the
organization will be obligated to hire safety auditors as part of protocols in
answering the legal notice.
Indirect cost. Repair or replacement of the wrapping machine as it is in the
vulnerable state for being used beyond the machine’s chart capacity.
Indirect cost. On the delay of production after the loaders do manual handling
as the only FLT is under repair resulting from the damage after overturning.
Indirect cost. The costumer might withdraw their order because of delay of
supply.
Indirect cost. Hiring cost of new workers to replace the ones who resigned due
to health issues.
2) UPLIFTMENT THE MORAL VALUE OF THE WORKERS AND THE ORGANIZATION.
There will be less issue on absenteeism and tardiness.
There will be less issue on turnovers as resignation of workers will be less.
The organization can also boast their high moral management system.
3) THE ORGANIZATION WILL BE FREED FROM THE BURDEN OF LEGAL NOTICES.
Consultation with the workers will lessen the burden of the organization from
legal notice; in contrary to the fact the organization has already received legal
notices from local bodies urging them to take action to prevent more injuries.
