 1. Does the “Job Step” column already cover all its steps that considered as necessary/critical?
Apakah kolom “Job Step” sudah mencakup semua langkah pekerjaan yang dianggap penting/kritikal?
 2. Have all the machines, equipment and tools contained/used in each “Job Step” been identified?
Apakah semua mesin, perlengkapan dan peralatan yang terdapat/dipakai di setiap “Job Step” sudah diindentifikasi?
 3. Have all the hazards been identified in each “Job Step”?
Apakah semua bahaya sudah teridentifikasi di setiap “Job Step”?
 4. Have all the control measures stated in the “Control Statement” column are sufficient/adequate (consider the hierarchy of control consist of: elimination, substitution, engineering control, administrative control,
& PPE)?

Chemical: H2S, Mercury, Acid, Benzene, Toluene, Xylene, Gas/Hydraulic/Fuel Pressure, Liquid, etc.
Biological: Virus, Snake, Germ, Microbe, Anthrax, Bacteria, etc.
Psychosocial: Stress, Worker Phobias, Fatigue, etc.
Ergonomic/Manual Handling: Mismatch, Awkward, Overreaching, etc.
