I have attached my class observation tool (COT) rating for limited face-to-face.
Successfully met the objective since I received the rating of ___ under indicator 1 for
applying knowledge of content across the teaching curriculum. This objective is vital to
delivering learning to the pupils. The transfer of it proves that my class met the desired

ANNOTATION
I see that research-based knowledge and teaching are evident in every part
of my lesson. This approach checks students' current understanding and
bridges the new task presented. Self-assessment, reflection, and analysis on
the effectiveness of the study skills used are also present.
As a teacher, I make sure that questions and instructions are both in Higher
Order Thinking Skills (HOTS) and Lower Order Thinking Skills with the
guidance of MELC.
The lesson employs feedback for positive criticism and allows to see what

proving that I have used effective verbal and non-verbal classroom communication
strategies that support learner understanding, participation, engagement, and
achievement. Effective communication is key to a well-managed classroom; whether
spoken or nonverbal plays an essential role in classroom organization without well-
developed skills of falling behind their peers or becoming overwhelmed or withdrawn from
school. Verbal communication establishes explicit instruction to the lesson and rules, while
nonverbal increases trust, clarity, and rapport.

MOV 6

ANNOTATION
For this objective, I have attained a rating of 7, indicating that objective was attained. In my
video presentation, I have mentioned that I used rubrics on questions that require personal
opinions. Lessons maintain a learning environment that promotes fairness, respect, and
care to encourage learning. Maintaining this environment provides a harmonious and
striving class that focuses on its common goal. I have attached the rubrics used to lessen
the subjectivity of the assessment. Rubrics are great for students. They let students know
what is expected of them, demystifying grades by clearly stating, in an age-appropriate
vocabulary.

❖ If option 1 and 2 are not possible, an

MOV 7
ANNOTATION
Set A
For set A, objective 7, I received 7. I maintain a learning environment that nurtures and
inspires learners to participate, cooperate, and collaborate in continued learning. It is in a
modular approach. Self-learning modules (SLM) have listed activities readily available. This
requires guidance not only for teachers but parents. Supplementary materials like videos
are also helpful. It creates a more engaging sensory experience than print materials alone.

MOV 7
ANNOTATION
SET B
For set B, objective 7, I received 7. I maintain a learning environment that nurtures and
inspires learners to participate, cooperate, and collaborate in continued learning. Though it
is limited to face-to-face, group work is discouraged, but the interaction between students
and teachers is precise and accurate. They can interact with each other and can direct
questions from the teachers. Questions are not difficult to answer since they can help each
different process new concepts. Here are some of the photos taken during my class
observation wherein students participate, collaborate, and cooperate.

MOV 8
ANNOTATION
For objective 8, Set A, the objective aims to apply a range of successful strategies that
maintain a learning environment that motivates learners to work productively by assuming
responsibility for their learning. With videos and audio, presenting students with problem-
solving lessons could approach critical thinking and creativity. Teaching your students how
to think critically and learn how to read between the lines and solve the problem is an
effective way for 21st Century learners based on modern technology and how they can use

MOV 8
it to learn more by themselves. Accompanying technology, you won't be teaching 21st-
century skills since many of them rely on digital tools.
ANNOTATION
For objective 8, I chose Set B. Here are some photos from my observation that show active
participation in my lesson. Evidence shows that from the executed MELC-based lesson
plan, learners' self-esteem, self-confidence, and self-awareness improve. Self-learning is an
essential skill for children as they journey through education and beyond to adulthood. For
this indicator, my rating is 7, which is an excellent indicator that I met the objective.

ACCOMPLISHMENT REPORT

Home visitation supports parents in better

understanding their child's development skills through
daily home activities.
The foundation of the teacher home visit program
rests on the school system's desire to increase student
academic achievement, increase parent involvement,
and motivate students to demonstrate more
positive behavior in the classroom.
The positive impact of this program is expected
because of the active participation of teachers, learners'
parents and guardians, volunteer parents/tutors, and
stakeholders. They provide us with their continued
support so our children/learners can cope well with
distance learning.
To achieve the school's primary goal is to help the
learners in distance learning, teachers work out different
interventions to address the problems identified to help
the pupils with their learning tasks and eventually
improve their skills. Even though we are considering the
health risk due to COVID 19 Pandemic, teachers of

Annotated Teaching Materials
ANNOTATION
The WHLP also trains learners to take responsibility for their own
learning as they follow the prescribed schedule in the plan. This is very
essential in this time of new normal education since it helps teachers,
learners, and parents to track the teaching-learning process.

MOV 14
ACTIVITY SHEET
ANNOTATION
It is equally important that each activity is meaningful, and ensures
student development and advancement through the unit. Activities
should build on previous activities and avoid being repetitive, they
should enable students to engage with and develop their skills,
knowledge and understandings in different ways.

MOV 14
ASSESSMENT TOOLS
ANNOTATION
As an assessment tool, worksheets can be used by teachers to
understand students' previous knowledge, outcome of learning, and
the process of learning; at the same time, they can be used to enable
students to monitor the progress of their own learning.

Personal Reflection notes

The Code of ethics for teachers is designed to protect the
rights of the students, all the students. Teachers must
understand that when they get a teaching position, they
agree to follow the Code of ethics.
A teacher's job is to provide quality education to all students.
A professional code of ethics must address this fact, stating

ANNOTATION
In this lesson, I used existentialism as the basis for planning
and designing my activities. The existentialist teacher is not
the center of the instruction but rather a facilitator. The goal
is to help students better understand who they are as
individuals. This also means that the student should have a
choice in what they learn and that the curriculum needs to be
somewhat flexible.
