AFFIDAVIT OF RENUNCIATION AND WAIVER OF RIGHTS


OF A PARCEL OF LAND

That I, APRESTO PEREZ, JR. of legal age, Filipino, married and residents of Nama,
Pozorrubio, Pangasinan and Las Piñas City, Metro Manila, after having been duly sworn to in
accordance to law hereby depose and say that;

That I am the only child of APRESTO PEREZ, SR. (deceased) and ARSENIA
PEREZ, now married, of legal age, Filipino and resident of Las Piñas City, Metro Manila and
grandson of POLICARPIO PEREZ and HERMILA JASMIN PEREZ, both deceased.

That I have acknowledged the extrajudicial settlement of estate with quitclaim executed
by the brothers and sisters of my father APRESTO J. PEREZ, SR. (deceased) bearing Doc. No.
______, Page No. ______, Book No. ______, Series of 2022, dated August 1, 2022, in the
Notarial Record of Atty. Francis Melville V. Tinio, Binalonan, Pangasinan, Philippines.

That my grandparents, POLICARPIO PEREZ and HERMILA PEREZ, when they are
still both alive, gave to me a parcel of agricultural land, located in Nama, Pozorrubio, Pangasinan
as my gift from them.

That I, APRESTO PEREZ, JR. of legal age, Filipino, by these affidavit, hereby
renounce, relinquish, and quitclaim all my share, participation, rights and interest of the parcel of
land described in the Deed of Extrajudicial Settlement of Estate with Quitclaim in favor of my
UNCLE RENATO PEREZ, AUNTIE’S REBECCA J. PEREZ, LILLIAN PEREZ-BATAS
and MERCEDES PEREZ-WENZEL.

That I executed this affidavit/instrument to attest the truth of all the aforesaid facts with
my own free will and voluntary act and deed.

IN WITNESS WHEREOF, I have hereunto set my hand this 31st day of August, 2022 at
Binalonan, Pangasinan, Philippines.
