That the examination was made – the following questions have been propounded by Atty.
Lizzette dela Peña – Dinglasan at her office at – and the answers made by the witness are made
under oath and that he/she may face criminal liability or perjury in lieu thereof.
OFFER OF TESTIMONY:
The testimony of the witness is being offered for the following purposes:
1. To prove the following:
1.1 The witness is the petitioner in this case;
1.2 The parties were married on –

1.4 Respondent is incapacitated to perform his marital obligation of giving love, respect,
fidelity, help and mutual support to his family;
1.5 Respondent is suffering from – making him psychologically incapacitated to fulfill
his/her fundamental marital obligations;
1.6 Respondent has been clinically diagnosed to be suffering ----,basic marital
obligations, a condition which is grave, severe and incurable and has juridical
antecedence making respondent psychologically incapacitated to comprehend and
comply with his basic marital obligations, for which parties’ marriage ought to be
declared void per Article 36, Family Code.
1.7 The parties did not acquire substantial properties;
2. To prove the material allegations in the Petition; and
3. To identify documents material to the Petition.
