BUBBLE – HE ASSESSMENT

POSTPARTUM
ASSESSMENT
Soft? Filling? Engorged? Nipples dry?
B - BREAST Intact? Note abnormalities such as
tenderness, pain, redness, lumps, &/or
cracked, bleeding, flat, inverted nipples.
Soft? Boggy? Firm? Location? Use these
U – UTERUS types of symbols to chart
(Fundus) Examples:
@U (at umbilicus)
1/U (I finger breath above umbilicus)
U/2 (2 finger breaths below umbilicus)

Note abnormalities such as deviated to the


L or R, boggy, firm with massage.
Note when the patient last had a normal
B – BOWEL bowel movement. Has the patient had a
BM since delivery? Document yes ( ) or no
( ). For all post-op patients: Assess bowel
sounds. Passing flatus? Abdomen soft, not
distended?
Has the patient voided since delivery? Is
B – BLADDER she voiding without difficulty? If she is
having difficulty describe what the problem
is, your interventions and evaluation. For
c-section patients, note patency of foley
cath, character and amount of urine, etc.
Check with the instructor before
discontinuing foley.
Note color, amount (examples: mod rubra,
L - LOCHIA scant serosa).
Note abnormalities such as excessive
amount of clots or foul odor.
Is the episiotomy clean, intact, edematous?
E – EPISIOTOMY Do “REEDA” assessment. Note presence
(Perineum) of hemorrhoids.

Incision: Note condition of abdominal


INCISION dressing. Initial dressings are removed by
the doctors.
Describe incision using “REEDA”
assessment.
