Heights, Brgy. Casili, Cebu City, do hereby name, appoint and constitute Mrs. ANALOU H.
BORJA, my mother likewise of legal age, married and presently residing at P-2 Don Julio, Brgy.
Canlanipa, Surigao City, Philippines as my true and lawful Attorney-In-Fact, for me and in my
name, place and stead for my own use and benefit, to do any or all of the following acts and
things to wit:
1. To receive the provident loan check due me; and
2. To sign any document necessary to have my check released.
