address at Pangil, Laguna, after having duly sworn to in accordance with law, hereby depose
and state;

That currently I am a Municipal Councilor of Municipality of Pangil, Pangil Laguna;

That I know personally RAM ROYCE P. SAMSON of Brgy. Natividad Pangil, Laguna;

That I am willing to take him and to be his custodian for his release under my
recognizance for the above entitled cases.

That I will guide him and assist him to comply with all the terms imposed by the decision
and likewise by the rules of his probation.

That I am committing to produce the person RAM ROYCE P. SAMSON, whenever the
Hon. Court so require.
