Jiarah Deleso Acay is my name. In the early hours of July 24, 2001 (4 AM) at Bontoc General
Hospital, I was born. I was born as a female. I didn't have twins; I was born alone. I was the third born
sibling out of all of my siblings. I had a birthweight of 3538 grams. My mother is Lalaine D. Acay. Her
name was Lalaine Clemente Deleso when she was a maiden. She is Filipino and practiced Anglican
religion. She was 29 years old and employed as a registered nurse when she gave birth to me. When I was
born, she and my father were residing in Upper Caluttit, Bontoc, Mountain Province. Mr. Danilo S. Acay
is my father. A Filipino, he is. He was a fireman at work. He practiced Anglican religion. He was 38 years
old when I was conceived.
In Baguio City, on December 29, 1995, my parents got hitched. The attending physician, a nurse,
and a midwife assisted my mother in giving birth. My birth certificate
was signed by the medial doctor, Vernice Padalla, MD, my father, Vera Grace Miranda, RN, and the
person at the civil registrar, Viola Okko.

II. List down 5 questions you have in mind about the contents of your birth certificate. What are
