AFFIDAVIT OF GUARDIANSHIP

I, LEONA L. DE GUZMAN, a Filipino, of legal age, and a resident of


Barangay Balele, Tanauan City, Batangas, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. I am the mother of LLOYD L. DE GUZMAN who was born on 22


August 1991;

2. My son is processing his requirements for his marriage on 24 July 2016;

3. However, his father, BALBINO DE GUZMAN, is currently working


abroad (Los Angeles) reason why the former cannot sign the documents needed for
our son’s application for marriage;

4. As his mother, I hereby accept and assume any and all responsibility for
his application for marriage, absent his father;

5. I am executing this affidavit to attest to the truthfulness and veracity of


the foregoing statements and for whatever legal intents and purposes it may serve

Affiant further sayeth naught.

IN WITNESS WHEREOF, I have hereunto set my hand this 1st day of July
2016 at Tanauan City, Batangas, Philippines.
