This is to certify that the abovementioned institution located at Region I, Diversion Road, San
Miguel, Calasiao, Pangasinan is exempted from using the Attendance Taking Module of
TESDA's Biometrics-Enabled Scholarship Registration System (BSRS) due to the following
reason/s:

 There is no available Internet Service Provider in the location and anywhere nearby,
including its neighboring residences upon inspection.

 Power Failure and temporary Issues Related to Internet Service Connectivity.

 With Religious and cultural reasons.

 Faces of scholars are withheld due to lawful order.

 Output-based modular classes conducted for short periods (less than 15 days) and in areas
where there are no economical means to have an internet connection

This Certificate may be revoked at any time by the Director-General upon the recommendation
of the Director IV, ICTO at any time.
