Electronic Equipment Borrowing Agreement


To borrow the school’s electronic equipment (TABLET WITH KEYBOARD Processor-octacore 2.0Ghz,
Display Size-10 inches, Display Resolution-800x1280 pixels, Camera-front 5mp and rear 13mp, Ram
Storage:Memory-4GB/ 64 GB battery6,000 mah Interface: USB Type C, 3.5mm audio-jack, Dual SIM Card
Slot Micro SD Card Slot Operating System: Android11 or higher with Google playstore color: Cyan Blue
brushed aluminum back plate with laminated SDO Tarlac Province Logo Accessories: Charger, Screen
Protector, Stylus Pen, User Manual, Magnetic Keyboard with Iron Gray Stand Folio Case with SDO Tarlac
Province Logo, earphones. Inclusions: USB OTG 32 GB, boot start logo of SDO Tarlac Province and
customized packaging for SDO Tarlac Province***12 months warranty), you must agree to the following
policy:
1. The tablet is Calibungan High School’s property. I agree to keep the equipment with me at all times. I
understand that I am personally responsible if it is lost, stolen, or damaged. I will pay all
repair/replacement costs resulting from loss (including theft) or damage of the equipment while it is
checked out in my name unless damage occurs during a building emergency such as fire.
2. I agree that the tablet is lent to me for academic purposes. I will report all equipment malfunctions or
problems immediately to the schools ICT coordinator.
3. I agree to return the equipment in good working order to the school ICT coordinator at the end of the
school year (S.Y. 2022-2023). The ICT coordinator will determine if the equipment is in good working
order or if it has been damaged.
4. I agree to save my files to removable media rather than the tablet’s hard drive. I understand that all files
saved on the tablet are erased when the tablet is returned to the ICT coordinator at the end of the school
year.
5. I agree to use ONLY the given power supply/charging unit with the tablet.
6. I will not tamper with the tablet’s hardware or software, or attempt to load software
7. I agree that failure to comply with any of these rules and guidelines will result in the loss of the privilege
of borrowing a tablet.
