AFFIDAVIT OF DISCREPANCY
I NENE SALAHUDDIN ADONA, of legal age, Filipino, widow and resident of Purok
Daisy, Upper Pulacan, Labangan, Zamboanga del Sur, after having been sworn to in
accordance with law hereby depose and say;
1. That I am the wife of FERNANDO L. ADONA JR, who died on July 21, 2021 at Kuta
Major Cesar L. Sang-An, Upper Pulacan, Labangan, Zamboanga del Sur;
2. That his name as appearing in our Marriage Certificate is PVT FERNANDO L. ADONA,
is due to oversight of the person who prepared the document in ascertaining his
complete name and is one and same person reflected on the Certificate of Live Birth,
Advisory on Marriages and Death Certificate which is correctly spelled and read as
FERNANDO LIWANAG ADONA JR;
3. That the name appearing in the Certificate of Live Birth of my child Hernando S. Adona
is FERNANDO LIWANAG ADONA, is inadvertence mistake by the clerk who prepared
the certificate of live birth and is one and same person reflected on the Certificate of Live
Birth, Advisory on Marriages and Death Certificate of my husband which is spell and
read as FERNANDO LIWANAG ADONA JR;
4. That the name appearing in the Certificate of Live Birth of my child Alvin S. Adona is
FERNANDO LIWANAG ADONA, is inadvertence mistake by the clerk who prepared the
certificate of live birth and is one and same person reflected on the Certificate of Live
Birth, Advisory on Marriages and Death Certificate of my husband which is spell and
read as FERNANDO LIWANAG ADONA JR;
5. That the name of my father-in-law appearing in our marriage certificate is FERNANDO
J. ADONA SR, is typographical error by the one who processed the documents, and is
one and same person reflected on the death certificate and advisory of marriages of my
husband which is correctly spell and read as FERNANDO GONZALEZ ADONA SR;
6. That the name of my father appearing in our marriage certificate is SALAHUDDIN
ALIPUDDIN, is inadvertence mistake by the clerk who prepared the marriage certificate
that have been interchange and is one and same person reflected on my birth certificate
and advisory on marriages which is spell and read as ALIPUDDIN JUHORA
SALAHUDDIN;
7. That my name appearing in our marriage is NENE SALAHUDDIN, is however correct
but there is no middle initial reflected and is due to miss look of the person who prepared
the marriage certificate and is one and same person reflected on the Certificate of Live
Birth and Advisory on Marriages which is correctly spell and read as NENE HASSAN
SALAHUDDIN;
8. That the name appearing in the birth certificate of my child Fernando S. Adona III is
SITTI NINIE SALAHUDDIN, is inadvertence typographical mistake by the clerk who
prepared the birth certificate and is one and same person reflected on the Certificate of
Live Birth and Advisory on Marriages which is correctly spell and read as NENE
HASSAN SALAHUDDIN;
9. That the name appearing in the birth certificate of my child Hernando S. Adona is NENE
SALAHUDDIN SALAHUDDIN, is inadvertence typographical mistake by the clerk who
prepared the birth certificate and is one and same person reflected on the Certificate of
Live Birth and Advisory on Marriages which is correctly spell and read as NENE
HASSAN SALAHUDDIN;
10. That the name appearing in the birth certificate of my child Alvin S. Adona is NENE
SALAPUDDIN SALAHUDDIN, is inadvertence typographical mistake by the clerk who
prepared the birth certificate and is one and same person reflected on the Certificate of
Live Birth and Advisory on Marriages which is correctly spell and read as NENE
HASSAN SALAHUDDIN;
11. That I am executing this Affidavit of Discrepancy in order to attest the truthfulness of the
foregoing facts, and for whatever legal purpose this affidavit may serve.
