Request Letter Assesor

This letter requests the separation of tax declarations for several properties owned by TCMPC cooperative. The cooperative had existing titles for land in Holy Spirit Village and Holy Spirit Village II Commercial Complex that were recently separated by the Registry of Deeds into individual titles. As a result of the title separations, the letter requests that the Provincial Assessor's Office also separate the tax declarations for the new individual titles. Documentation including copies of titles and subdivision plans are provided to support the request.

Good day!

Our cooperative- TCMPC has an existing subdivision project named HOLY SPIRIT VILLAGE
located in Brgy. Mataywanak Tuy, Batangas with Transfer Certificate of Title No. 055- 2016001548
described as Lot 5-B of the approved subdivision plan (LRA) PSD-04-249853, and HOLY SPIRIT
VILLAGE II Commercial Complex with Transfer Certificate Title No. 055-2019001603, Title No. 055-
2019001608, Title No. 055-2019001650 of the approved subdivision plan (LRA) PSD-E2020003649,
PSD- 04-258428 and PSD-04-258431.

We have requested Registry of Deeds of Nasugbu, Batangas for the separation of these titles and
was approved and already released, for TCT No. 055- 2016001548 with 118 titles of approved
subdivision plan PSD-04-249853 and for TCT No. 055-2019001603, 055-2019001608 and 055-
2019001650 with 11 titles with approved subdivision plan PSD-E2020003649, PSD- 04-258428 and
PSD-04-258431.

In line with this, may we request your good office for separation of the tax declaration of these
titles. Attached herewith are the following documents:

1. Photocopies of TCT NO. 055- 2016001548, 055-2019001603, 055-2019001608 and 055-2019001650


2. Photocopies of 118 separated titles from TCT No. 055- 2016001548, 4 separated titles from TCT No.
055-2019001603, 4 separated titles from TCT No. 055-2019001608 and 3 separated titles from TCT No.
055-2019001650;
3. Copy of the approved subdivision plan PSD-04-249853, PSD-E2020003649, PSD- 04-258428 and
PSD-04-258431.
4. Copy of Certificate of Tax Exemption of our cooperative from Bureau of Internal Revenue
