Violence Assessment Tool

This document contains an assault and violence assessment tool used to evaluate patients. It assesses key risk factors such as history of violence, substance abuse, paranoia, and impulsivity. For each factor, the patient is given a score of 0, 1, or 2 points depending on the level of risk.
