This document contains an evaluation sheet for a student's 80-hour work immersion at a food processing company. It evaluates the student on competencies such as teamwork, communication, attendance, initiative, dependability, attitude, and professionalism. For each competency there are descriptive indicators of performance rated on a scale of 1 to 5. An overall rating is given which is then translated to an average grade based on a scale of interpretation. The evaluation sheet is signed by the student, school partner, principal, industry focal person, and company owner to certify the evaluation.
