Case of Gonzales, Noel 39 y/o male, diagnosed case of CKD secondary to Chronic Glomerulonephritis
since February, 2019, he is on a regular 3x a week hemodialysis treatment in this institution.

PHYSICAL EXAMINATION:
Conscious, coherent, not in respiratory distress
BP 140/90mmHg, CR – 81bpm, RR – 21cpm
Pale palpebral conjunctiva
Clear lung fields
Abdominal: soft, non-tender
Lower Extremities (-) edema
PMHx: (+) Hypertension

RT PCR (+) – January 07, 2021

DIALYSIS PRESCRIPTION:

Dialyzer - 18-19 High Flux


Qb - 250-300ml/min
Qd - 500ml/min
Heparin` - Regular
Dry Weight - 52.85kgs
Bath - 2k

MEDICATIONS:
1. Losartan 100mg/tab OD
2. Amlodipine 10mg/tab OD
3. Calcium Carbonate 500mg/tab TID
4. Sodium Bicarbonate 650mg/tab TID
5. Ferrous Sulfate + FA 1tab TID
6. Epoetin Alfa 4,000 SC every after HD
