AFFIDAVIT

I, MERLINA F. CATAPANG, Filipino, married, of legal age and a resident of Blk


1 Lot 26 Country Drive, Country Homes, Soro Soro Biñan Laguna, after having been
sworn in accordance with law do hereby depose and state that:

1. I am a Vice-President of Philend Lending Investors, Inc., the registered


owner of a 2017 Honda BRV with plate number NCM 1922.

2. As Vice-President of Philend, I was provided with the above-mentioned


vehicle as a company car.

3. Last April 9, 2022, at around 9:00 in the morning, I parked the said vehicle
under a mango tree, near our residence, as I usually do. There was no damage to the
windshield of the vehicle.

4. After one hour, when I was about to leave again, I noticed that the
windshield was already damaged at the bottom part of the driver’s side.

5. I also noticed that there were a few fallen mango fruits near the said
vehicle. It appears that a mango fruit fell onto the windshield which caused the said
damage.

6. This Declaration is executed to prove the truth of the foregoing statements


for whatever legal purpose this may serve.
